It gives patients with physical, behavioral and cognitive impairments access to comprehensive therapy in multiple settings. Responsible for direct patient care, housekeeping, cooking, carrying out assigned nurse delegation tasks, engaging the patient in day-to-day activities of daily living, and therapist-directed activities.</p>\n<p> </p>\n<ul>\n <li>Follow through on the patient plan of care in collaboration with other transdisciplinary team members, including assisting in rehabilitation activities assigned to promote education, and maintaining patient engagement.</li>\n <li>Assist and help patients achieve appropriate goals in all environments, including home, therapy, and community.</li>\n <li>Assist patients in meal planning, food preparation, and assisting with grocery shopping. Prepares grocery and facility inventory. Carry out suite management and overall housekeeping tasks as assigned.</li>\n <li>Insure the safety and wellbeing of patients, responsible to review weekly residential recommendations and follow through on assigned level of supervision required for each patient.</li>\n <li>Perform other duties as assigned.</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Minimum Qualifications:</strong></p>\n<ul>\n <li>Current high school diploma or equivalent</li>\n <li>BLS certification through the American Heart Association required</li>\n <li>Current State Driver's License in good standing required</li>\n</ul>\n<p><strong>Preferred Qualifications:</strong></p>\n<ul>\n <li>Two years of experience in an assisted living or rehabilitation environment preferred; experience with brain injury population preferred</li>\n <li>Ability to delegate and problem solve effectively.</li>\n</ul>\n<h2>Additional Data</h2>\n<p><em>Equal Opportunity Employer/including Disabled/Veterans</em></p>",
