    "requisitionDescription": "<div><h1 data-pasted=\"true\">JOB SUMMARY</h1><p>This position is responsible for outreach to Veterans and individuals in the community that are in need rapid re-housing and homeless prevention services. Referring persons served in need of housing services to available resources. Initial assessment of housing support services including conducting strength-based needs assessment.</p><p><strong>ESSENTIAL DUTIES AND RESPONSIBILITIES:</strong></p><ul type=\"disc\"><li>Outreach services between 12-8:30pm&nbsp;</li><li>Connect individuals with benefits and supports of case management team and intake/data specialist</li><li>Communicate effectively with Outreach Supervisor regarding community needs and direction towards coverage of the 16 parishes VOASELA cover.&nbsp;</li><li>Conduct outreach in the 16 parishes VOASELA cover via landlord communication and connections to that region resources and homeless shelters</li><li>Verify homelessness of veterans and individuals in the community that are in need of services.</li><li>Provide support and direct assistance to tenants based on an individualized housing support plan and engagement of resources in the 16 region parishes we cover&nbsp;</li><li>Assist with Transportation, communication, and self-advocacy as needed for Veterans and individuals in the community</li><li>Assist other team members as needed.&nbsp;</li><li>Advocate for veterans&rsquo; services in problem-solving conflicts and advocate including people who are served in the community &amp; need VOASELA referrals and resource engagement</li><li>Attend Stand-Down events and other community organized events as scheduled</li><li>Connect with the local VA offices and GPD sites for assistance as needed. This also includes local CoC, shelters, street outreach and visits to all housing aspects including but not limited to apartments/facilities etc. &nbsp;</li><li>Visit the Community Resource Referral Center as scheduled by Outreach Supervisor</li><li>Perform other duties as assigned</li></ul><p><strong>ADDITIONAL DUTIES AND RESPONSIBILITIES (Non-essential)</strong></p><ul><li>Adhere to the designated dress code as stipulated for the specific role.</li></ul><p><strong>SUPERVISORY DUTIES:</strong></p><ul><li>None</li></ul><p><strong>KEY CONTACTS</strong></p><p>Maintains critical relationships with the following:</p><ul><li>Serves as primary representative of the Agency with respect to third party vendors providing training-related products and services</li><li>Utilizes the Relias Learning Management System to maintain compliance with all required training courses</li></ul><p><br></p><div class=\"fr-page-break\" contenteditable=\"false\" style=\"break-after: page;\" data-title=\"PageBreak\"><div class=\"fr-page-break-line\"><strong><span class=\"fr-page-break-label\">Page Break</span></strong></div></div><p><br></p><p><strong>KNOWLEDGE, SKILLS AND ABILITIES:</strong></p><p>This position requires knowledge, skills and abilities equivalent to:</p><ul><li>High school diploma required</li><li>Community Health workers (CHW) preferred certification</li><li>Must self-identify as have a disability and receive or currently receiving services</li><li>Demonstrated basic writing skills</li><li>Demonstrated skills necessary to engage a diverse group of individuals</li><li>Demonstrated ability to work in a team environment.</li></ul><p><strong>Veterans are preferred for this position.&nbsp;</strong></p><p>Must have:</p><ul type=\"disc\"><li>Current driver&rsquo;s license issued by state of residence</li><li>Current Vehicle Inspection decal</li><li>Current vehicle insurance</li><li>Current vehicle registration in state of residence</li><li>Satisfactory Motor Vehicle Report</li><li>Must be able to pass a criminal history investigation</li></ul><p><strong>PHYSICAL REQUIREMENTS:&nbsp;</strong></p><p>The work environment characteristics and physical demands described here are representative of those an employee encounters while performing the essential functions of this job.
