You’re looking in the right place.</p>\n<p> </p>\n<p>As a <strong>Physiotherapist</strong>, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners. </p>\n<p> </p>\n<p>Whether you work in our office, in the community, or a health care facility – you will play a lead role in providing connected, accessible, patient-centered care – and be supported by our collaborative team that includes over 9,000 regulated health care and other professionals.</p>\n<p> </p>\n<p>As a valued team member, your mission will be to help our patients be healthier at home, while you benefit from our supports for professional growth, personal wellness and work-life balance.</p>\n<p> </p>\n<p><strong>What will you do?</strong></p>\n<p>The Physiotherapist plans, organizes and directs diverse programs of treatment, counseling, screening and education for ONTARIO HEALTH ATHOME Champlain Patients in a variety of settings (in homes and retirement homes)</p>\n<p> </p>\n<p><strong>Primary Responsibilities:</strong></p>\n<ul>\n <li>Provides physiotherapy services to ONTARIO HEALTH ATHOME Champlain patients through initial assessment, identification of problems, establishment of goals and treatment plans, implementation of physio treatment, follow-up and management of patient’s condition.</li>\n <li>Establishes priorities and treatment goals with respect to identified problems and develops methods of achieving these goals.</li>\n <li>Ensures that clinical records are maintained in accordance with ONTARIO HEALTH ATHOME Champlain Policies and Procedures and the College of Physiotherapists of Ontario.</li>\n <li>Schedules patient visits according to Physiotherapy priorities.</li>\n <li>Functions as a member of a multi-disciplinary team and maintains liaison with other health care professionals.</li>\n <li>Acts as an education and resource person for other health care professionals.</li>\n <li>Participates in committees as approved.</li>\n <li>Partakes in professional development activities and maintains knowledge and ability in the realm of physiotherapy.</li>\n <li>Maintains supplies and recommends equipment for department use within budget limitations.</li>\n <li>Interprets the role and objectives of the ONTARIO HEALTH ATHOME Champlain Physiotherapy Program in the community.</li>\n</ul>\n<p><strong>Team/Department: </strong>Allied Health</p>\n<p><strong>Hours of work: </strong>Casual hours between 8:30am-4:30pm – Monday to Friday</p>\n<p><strong>FTE:</strong> 1.0</p>\n<p><strong>Starting Salary: </strong>$44.480 to $51.340</p>\n<p><strong>Affiliation: </strong>ONA</p>\n<p><strong>Reporting to: </strong>Manager, Patient Services </p>\n<p><strong>Office Location: </strong>Labelle</p>\n<p> </p>\n<p> <strong>What must you have?</strong></p>\n<ul>\n <li>Degree or diploma in Physiotherapy and current certificate of registration with the College of Physiotherapists of Ontario</li>\n <li>Related professional experience which would normally be attained over a period of two years in a Physiotherapy role</li>\n <li>Related experience in a similar community-based setting is an asset</li>\n <li>Current CPR-C certificate</li>\n <li>We have a mandatory COVID-19 vaccination policy.
