    "description": "<p>YHDP<br>A Program of Comprehensive Youth Services, Inc.<br>Position: Full time Case Manager<br>Qualifications: The Full –time Case Manager must have a bachelor&amp;#39;s degree with a<br>major in criminal justice, psychology, social work or guidance and counseling, or<br>actively pursuing the above mentioned degrees. Must have a valid Driver&amp;#39;s License<br>a clean driving record, must pass all required MDHHS background checks.<br>Experience: Experience and knowledge of youth development, experience in advocacy<br>and case management and experience in working with youth is desired.<br>Job Duties: The position requires a positive youth development and trauma informed<br>care approach, complete or assist in the completion of the Case Manager and Outreach<br>duties, as assigned.<br> Assist in the completing Intake/assessments, case management, and required<br>paperwork in a timely manner<br> Overseeing the completion of the client Educational referrals<br> Answering the agency crisis and information/referral calls<br> Participate in agency Intrastaff and monthly staff meetings<br> Provide individual and family Case Management and assistance in accessing<br>Community resources and housing assistance<br> Complete Client Care Monitoring for agency files minimally once a month<br> Oversee the completion of aftercare and follow-ups with exited clients <br> Follow all required local, state, and federal licensing requirements <br> Assist in the implementation of the programs structure <br> Compiles data and narrative descriptions for quarterly, semi-annual and<br>annual reports as assigned.<br> Assist with RHYMIS and HMIS data entry and reporting <br> Complete all required annual training hours<br> Participate in Community Coalitions and Networks as applicable<br> Other duties as assigned by the Program Manager<br>Hours: To be determined by the Program Manager or Director of RHY based on youth<br>and program needs, availability for some weekends and some holidays as needed</p>\n<p><br></p>\n<p><br></p>",
