It’s the quiet moments, the hard conversations, and the privilege of being there when it matters most.</p>\n<p>Now imagine taking that experience and stepping into a role where you can guide a team, support your fellow nurses, and shape the care patients and families receive every single day.</p>\n<p> </p>\n<p>Good Shepherd Hospice is looking for a <strong>Patient Care Manager</strong>, and a nurse with <strong>3–5 years of hospice experience</strong> could be the perfect fit.</p>\n<ul>\n <li>Still close to patient care</li>\n <li>Growing into leadership</li>\n <li>Making an even bigger impact</li>\n</ul>\n<p> </p>\n<p>If you’ve been thinking about “what’s next” in your career… this might be it.</p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Coordinates the development, implementation and review of the Plan of care for each patient/family. </li>\n <li>Steps in as the Alternate Administrator in the absence of the Administrator/Executive Director. </li>\n <li>Leads and supervises weekly Interdisciplinary Team meetings.
