Description
SUMMARY:
Under the supervision of the Program Director, the Healthcare Navigator works to enroll eligible individuals into health insurance, in order to maximize opportunities to bill for services. They develop educational material regarding benefits and prepares and delivers trainings regarding insurance benefits to clients, at community events, health fairs, meetings, and other venues.
DUTIES AND RESPONSIBILITIES:
Performs key functions related to the success of the MAT program within Four Corners including patient outreach via telephone, community outreach and office encounters, coordination of patient activities, and the appropriate utilization of services. Educates colleagues, clients, loved ones, and partners on healthcare systems and insurance benefits. Maintains appropriate training and documentation for successful third party reimbursement. Works in conjunction with case managers and client engagement advocate to follow any changes with clients treatment/recovery needs. Ensures clients are aware of rights, responsibilities, and take full advantage of benefits throughout their treatment and recovery. Coordinates services with case managers, the jails, the courts and system partners. Stays abreast of changes to laws locally and nationally. Maintains ethical and confidentiality rules and regulations in accordance with HIPAA/42 CFR Part 2. May be required to perform other duties as required.
