CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community.</em></p><p><br></p><p><strong>POSITION SUMMARY:</strong></p><p>The Heroin Overdose Prevention Education (HOPE) program provides an opportunity for individuals involved with the criminal justice system who are charged with an offense and exhibit substance use to access care and treatment. The long-term goal of the program is to support participants in managing their lives in the community and prevent future arrests and returns to incarceration.</p><p>The Recovery Counselor serves as a key clinical member of the HOPE program team, providing substance use disorder assessments, individual and group counseling, crisis intervention, treatment planning, and recovery support services to justice-involved participants. The Recovery Counselor works collaboratively with Senior Peer Mentors, court personnel, and community partners to support participant engagement and continuity of care.</p><p><br></p><p><strong>DUTIES &amp; RESPONSIBILITIES</strong></p><ul><li>Conduct comprehensive substance use disorder screenings, assessments, and evaluations for individuals referred through the Richmond County District Attorney’s Office, courthouse engagements, and community-based referrals</li><li>Develop individualized action plans and treatment recommendations using trauma-informed and person-centered approaches</li><li>Provide individual and group counseling using evidence-based interventions focused on substance use, relapse prevention, and recovery support</li><li>Facilitate recovery-oriented skill building and psychoeducational group activities</li><li>Provide crisis intervention and de-escalation support to program participants as needed</li><li>Conduct follow-up and aftercare calls to support sustained engagement and recovery</li><li>Coordinate referrals and linkages to treatment, ancillary, and supportive services</li><li>Escort and accompany participants to treatment providers, court appearances, and community-based services as needed</li><li>Participating in weekly case review meetings with court partners and other external stakeholders</li><li>Provide guidance and support to Senior Peer Mentors related to participant engagement and service coordination</li><li>Complete all required program documentation accurately and in a timely manner using Salesforce and other approved systems</li><li>Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values</li><li>Other duties as requested or at the needs of the program and/or agency</li></ul><p><em>The above is intended to describe the essential job functions, the general supplemental functions, and the essential requirements for the performance of the job; it is not to be construed as an exhaustive statement of all the job functions.</em></p>",
