Serving approximately 20,573 annually and providing 1,856 units of supportive housing last year, LOFT is one of Ontario’s largest mental health service providers of its kind. Job Summary: The Team Lead, Personal Support Services is responsible for a range of services for individual client members. The Team Lead, Personal Support Worker will provide direct support to clients in a psycho social (recovery) service approach, assist, in liaison with the Program Manager and the staff team with the implementation and maintenance of all services and perform administrative tasks related to the operation of the program. There will be an expectation to actively contribute to the program’s staff team and to adhere to the established routines or protocols of the program including emergency, security, medication, fire and general health and safety practices. This position may involve work on weekends. Bradford House provides specialized assisted living support to vulnerable older adults with complex needs. Bradford House responds to each resident’s needs, so they can live safely and in good health in the community as they age. Through our care, we aim to postpone for as long as possible, or eliminate, any need for institutional care.Our program offers 24/7 care through our specialized Personal Support Worker (PSW) team, providing such services as assistance with personal care, essential housekeeping, laundry services, medication support and escorts to appointments. LOFT’s Case Managers offer one-on-one support and provide care coordination, crisis intervention, system navigation and client advocacy. They connect clients with the services they need to become healthier, happier, and as independent as possible. Our team calls upon a variety of resources, including medical, financial, emotional and recreational supports.Mental, physical and spiritual health are improved by social interaction and the sense of belonging that comes from a welcoming and supportive community. Our life enrichment services offer residents opportunities to enjoy life, try new things, and become involved in their local community. The goal is to ensure that clients have access to interesting and stimulating activities and social opportunities through regular in-house events and group outings in the community.Bradford House offers healthy and delicious meals and snacks for residents and is able to accommodate certain special diet requests. What You Will Do: Support of the physical and mental health requirements of client members Establish a working rapport with client members that supports their dignity, self-direction and facilitates their psychosocial, behaviour management and medical needs, as required. Monitor mental health related behaviours and provide support as required. Identify changes in behaviour and/or other indicators of decompensation with clients’ mental health challenges. Provide input into team planning to manage clients’ mental health issues. Take the lead role and intervene in clients’ concerns related to both mental and physical health. Identify and report any incidents of suicide ideation among clients; ensure a safe environment for the client to reduce the risk of self-harm. Facilitate conflict resolution between clients with psychiatric and cognitive disabilities. Support and monitor clients’ psychotropic medications and notify appropriate mental health/medical professionals regarding related issues as required. In liaison with the Program Manager, assist with monitoring and maintaining client members’ appointments with physicians, community agencies, support services and other services, as required. Support client members with reminders related to taking their medications at the scheduled time(s) and in accordance with their physician’s instructions, as required. Monitor and support client members with medical problems and illnesses, including cleaning areas which require immediate attention, i.e. linens, floor coverings, etc. for the comfort and support of the members. Further, to record illnesses, the circumstances and arrange for medical/emergency intervention in liaison with the Program Manager. Maintain and record all appointments and hospital admissions for client members. In liaison with the Program Manager, assist with the coordination of all aspects of the admission process of new client members as well as the discharge procedures for those who leave the program. This includes the input of client information into the data/TREAT system as applicable. As a means of promoting a normalized lifestyle, support client members with daily living chores such as bed making, dusting, sweeping and generally keeping their living space in a clean and orderly fashion. Provide support with managing behaviours arising from cognitive issues. Provide crisis intervention and manage emergency situations, according to policies and procedures of LOFT Community Services. Provide social support in areas such as conversation, making phone calls, writing letters, facilitating interest in hobbies and other recreational activities Escort to medical appointments, visiting friends and other identified needs for social interaction in the community. Maintain current and accurate records of the provision of care services and document behaviours for individual client members in accordance with established procedures. Participate in regularly scheduled meetings with client and/or other members of the staff team for the purpose of developing goals of support for individual client members, as required. Adhere to the goals and tasks developed. Report and consult with the Program Manager in regard to special incidents within 24 hours. In liaison with the Program Manager, provide ongoing assessment and monitoring of the client members’ requirements for support services and assistance in coordinating and arranging for the delivery of these services, as required. Maintenance of security, fire and general health and safety In emergency situations including injuries and medical health concerns, contact appropriate emergency services immediately. In addition, record all incidents using the appropriate forms. Contact with the Program Manager or designated person will be initiated in accordance with program procedures and practices. If a client member is missing, follow up according to the program procedures. Ensure equipment and supplies for personal support staff are readily available. Report any damage or need for maintenance to the Program Manager. Social/Recreational Activities Participate in the development, implementation and evaluation of recreational and social programming designed to maintain and enhance physical well-being and cognitive function and to promote a normalized lifestyle. Purchase supplies for the programs, as required, as well as assist with preparing posters, etc. to publicize activities. Maintain accurate records of program participants. Keep appropriate staff informed of clients absent for other scheduled activities (e.g. meals) because of special outings, as applicable. Ensure the equipment/supplies required are available and ready for use. Report any damage/need for maintenance of equipment to the Program Manager. Actively participate in all planned program activities. Actively encourage positive interactions among participants. Drive the van to transport client members for outings as well as assume the leadership for the activity, as required. Other related duties and delegated responsibilities from the Program Manager Participate in client members’ activities and meetings, staff meetings, training sessions and the Occupational Health and Safety Committee, as required. Prepare the work schedule for PSW staff at the Bradford House site. Accept other delegated assignments from the Program Manager which pertain to the overall operation of the Bradford House Supportive Housing Program What You Bring: Completion of a post secondary education in Human Services (ie: PSW certificate or other related college diploma), or possesses a current and valid RPN designation from a recognized institution that meets the standards of the province of Ontario and MOHLTC; Experience working with vulnerable, older persons and seniors with mental health issues, addictions and other special needs. Ability to organize activities and work with individuals and groups in the areas of assessment, and problem solving Ability to intervene and manage crisis situations Knowledge and proven experience working with clients in a PSR (Psychosocial Rehabilitation) approach; Ability to organize and work with individuals and groups in the areas of assessment, problem solving and crisis management; Ability to interact with clients in a positive and supportive manner; Physically able to perform demanding body mechanics (i.e. lifting, transferring, etc.); Basic knowledge of medications and general health conditions First Aid and CPR certification Excellent verbal and written skills in English language; Excellent computer skills; Ability to work effectively in a team environment; Ability to work in a multicultural environment Ability to interact and communicate effectively with special needs seniors, multi-disciplinary professionals and other staff Knowledge of social services and community services for seniors Possession of a valid Ontario Driver’s License preferred Ability to communicate in a different language, an asset. Understanding of cultural competency and anti-oppression framework. Further, to record illnesses, the circumstances and arrange for medical/emergency intervention in liaison with the Program Manager.\n Maintain and record all appointments and hospital admissions for client members.\n In liaison with the Program Manager, assist with the coordination of all aspects of the admission process of new client members as well as the discharge procedures for those who leave the program. Further, to record illnesses, the circumstances and arrange for medical/emergency intervention in liaison with the Program Manager.</span></span></li><li><span style=\"font-size:14px;\"><span style=\"font-family:Tahoma, Geneva, sans-serif;\">Maintain and record all appointments and hospital admissions for client members.</span></span></li><li><span style=\"font-size:14px;\"><span style=\"font-family:Tahoma, Geneva, sans-serif;\">In liaison with the Program Manager, assist with the coordination of all aspects of the admission process of new client members as well as the discharge procedures for those who leave the program. This includes the input of client information into the data/TREAT system as applicable.</span></span></li><li><span style=\"font-size:14px;\"><span style=\"font-family:Tahoma, Geneva, sans-serif;\">As a means of promoting a normalized lifestyle, support client members with daily living chores such as bed making, dusting, sweeping and generally keeping their living space in a clean and orderly fashion.</span></span></li><li><span style=\"font-size:14px;\"><span style=\"font-family:Tahoma, Geneva, sans-serif;\">Provide support with managing behaviours arising from cognitive issues.</span></span></li><li><span style=\"font-size:14px;\"><span style=\"font-family:Tahoma, Geneva, sans-serif;\">Provide crisis intervention and manage emergency situations, according to policies and procedures of LOFT Community Services.</span></span></li><li><span style=\"font-size:14px;\"><span style=\"font-family:Tahoma, Geneva, sans-serif;\">Provide social support in areas such as conversation, making phone calls, writing letters, facilitating interest in hobbies and other recreational activities</span></span></li><li><span style=\"font-size:14px;\"><span style=\"font-family:Tahoma, Geneva, sans-serif;\">Escort to medical appointments, visiting friends and other identified needs for social interaction in the community.</span></span></li><li><span style=\"font-size:14px;\"><span style=\"font-family:Tahoma, Geneva, sans-serif;\">Maintain current and accurate records of the provision of care services and document behaviours for individual client members in accordance with established procedures.</span></span></li><li><span style=\"font-size:14px;\"><span style=\"font-family:Tahoma, Geneva, sans-serif;\">Participate in regularly scheduled meetings with client and/or other members of the staff team for the purpose of developing goals of support for individual client members, as required.
