    "requisitionDescription": "<div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><div><p id=\"isPasted\"><strong>Great Life work Balance position with excellent benefits!&nbsp;</strong></p><p>The Psychiatric Mental Health Nurse Practitioner is responsible for providing comprehensive and high quality behavioral health services to the complex and diverse older adults, including those who may reside in assisted living facilities &amp; skilled nursing facilities. &nbsp;Incumbent delivers behavioral health care &amp; manages a caseload of clients referred by the individual&rsquo;s PCP. <strong>This is a full-time position, Monday- Friday 8-4 position: no nights, weekends or holidays! &nbsp;Covering Beverly and Gloucester sites.</strong></p><p>&nbsp;<strong>Responsibilities:</strong></p><ul type=\"disc\"><li data-pasted=\"true\">Performs comprehensive initial behavioral health evaluations for complex older adults in a variety of care settings including day center, the individual&rsquo;s home, assisted living facilities, or skilled nursing facilities.</li><li>Evaluates participants for acute changes in behavioral health status to mitigate risk of psychiatric hospitalization and provides close follow-up from psychiatric hospitalizations.</li><li>Makes nursing home visits in accordance with the state nursing home requirements.</li><li>Makes home visits to participants as needed.</li><li>Follows up with participants routinely or urgently as clinical need arises.</li><li>Develops clinically appropriate behavioral health treatment plans and follows up regularly on medication regimens.</li><li>Implements gradual dose reductions (GDR) as needed and in line with regulatory practices.</li><li>Monitors clients&rsquo; response to psychotropic medications and adjusts treatment plans as warranted.</li><li>Documents evidence based and clinically appropriate treatment interventions with strong emphasis on management of psychopharmacology.</li><li>Participates in behavioral health initiatives to enhance clinical practice, teamwork and program development.</li><li>Covers for Behavioral Health colleagues when needed.</li><li>Provides appropriate behavioral health teaching to participants, families, caregivers, &amp; the Interdisciplinary Team.</li><li>Adheres to the standards set forth in the Element Care NP/MD Collaborative Practice Guide.</li><li>Completes appropriate, accurate, and timely documentation.</li><li>Ensures that duties are accomplished in a fiscally responsible manner adhering to established budgetary constraints.</li><li>Ensures that all interactions with participants are conducted in a manner that adheres to the highest standards of care for all Element Care clients.</li><li>Ability to pass an N95 mask fit test. Position requires mask where seal is critical. Candidate is required to not have facial hair that interferes with a tight seal of the respirator.</li><li>Frequent local travel.</li><li>Performs other duties as required.</li></ul><p><strong>Qualifications &amp; Skills:</strong></p><p><strong>Job Specifications:</strong></p><ul type=\"disc\"><ul data-pasted=\"true\"><li>Masters of Science in Nursing (MSN).</li><li>Current Board Certification as a Psychiatric Mental Health Nurse Practitioner.</li><li>Current licensure as a Nurse Practitioner in Massachusetts.</li><li>Current Massachusetts Controlled Substances Registration and DEA Prescription Privileges, current CPR Certification.</li><li>At least five (5) years of experience working with a geriatric population required. &nbsp;</li><li>At least one (1) year of experience working with low income and diverse cultural groups preferred.</li><li>Previous skilled nursing facility experience preferred.</li><li>Obtains and maintains appropriate contracted facility credentials and privileges.</li><li>Demonstrated ability to work within urban communities with strong emphasis on racially, ethnically, and culturally diverse, low income, and special needs populations.<ul><li>Ability to work well within a team concept and to be respectful of coworkers and colleagues.</li><li>Ability to communicate clearly and succinctly with other team members.</li></ul></li></ul><li><br><ul type=\"disc\"><li>Ability to be able to quickly recognize situations/problems before they become acute, and to be able to implement effective solutions and understand consequences. &nbsp;</li><li>COVID vaccinated preferred.</li></ul></li></ul><p><strong>Benefits:</strong></p><ul type=\"disc\"><li>Health insurance</li><li>Dental insurance</li><li>Vision insurance</li><li>Paid time off</li><li>Retirement plan</li><li>Supplemental benefits</li></ul><p>&nbsp;</p><p><em>EEO Statement</em></p><p><em>Element Care is an Equal Opportunity Employer.
