The care navigator is responsible for assessment, referrals, linking and coordination of all services, the creation of an individualized plan of treatment, and monitoring authorized services. The care navigator is the primary case-holder for the clinical record. This position is a community-based position.</span></span></p>\n\n<p>&nbsp;</p>\n\n<p><span style=\"font-size:12pt\"><strong><u><span style=\"font-size:11pt\">EDUCATION AND EXPERIENCE:</span></u></strong></span></p>\n\n<ul>\n\t<li><span style=\"font-size:12pt\"><span style=\"font-size:11pt\">Must meet the criteria as a Child Mental Health Professional (CMHP) and or a Qualified Intellectual Disability Professional (QIDP) or Qualified Mental Health Professional (QMHP).&nbsp; </span></span></li>\n\t<li><span style=\"font-size:12pt\"><span style=\"font-size:11pt\">Minimum of a Bachelor&rsquo;s degree in Social Work, Psychology, or a mental health-related field from an accredited school is required.</span></span></li>\n\t<li><span style=\"font-size:12pt\"><span style=\"font-size:11pt\">Must be trained and have a minimum of 1 - 3 years of supervised experience in the examination, evaluation, and treatment of minors and their families, or adults is required.</span></span></li>\n\t<li><span style=\"font-size:12pt\"><span style=\"font-size:11pt\">Prior experience working in a Community Mental Health setting is preferred.</span></span></li>\n\t<li><span style=\"font-size:12pt\"><span style=\"font-size:11pt\">A current valid professional licensure/certification in the State of Michigan is preferred.</span></span></li>\n\t<li><span style=\"font-size:12pt\"><span style=\"font-size:11pt\">Experience working in the home and/or community setting is ABILITIES:</span></u></strong></span></p>\n\n<ul>\n\t<li><span style=\"font-size:12pt\"><span style=\"font-size:11pt\">Knowledge of various systems operating in Wayne County (i.e. Community Mental Health, Department of Social Services, Juvenile Court, Probate Court, School).</span></span></li>\n\t<li><span style=\"font-size:12pt\"><span style=\"font-size:11pt\">Skills in working with children, adolescents, adults and their families.</span></span></li>\n\t<li><span style=\"font-size:12pt\"><span style=\"font-size:11pt\">Ability to elicit assistance from various systems to provide individual services to clients.</span></span></li>\n\t<li><span style=\"font-size:12pt\"><span style=\"font-size:11pt\">Ability to negotiate alliances among various agencies to establish trust and to articulate mutual interests. </span></span></li>\n\t<li><span style=\"font-size:12pt\"><span style=\"font-size:11pt\">Knowledge and experience using an electronic client medical record.</span></span></li>\n\t<li><span style=\"font-size:12pt\"><span style=\"font-size:11pt\">Ability to work cooperatively with all program staff and support services setting.</span></span></li>\n\t<li><span style=\"font-size:12pt\"><span style=\"font-size:11pt\">Develop/maintain liaison relationships with various community facilities.</span></span></li>\n\t<li><span style=\"font-size:12pt\"><span style=\"font-size:11pt\">Develop, per agency policy, a written client comprehensive assessment and service plan, which identifies the needs of the client/family.</span></span></li>\n\t<li><span style=\"font-size:12pt\"><span style=\"font-size:11pt\">Assist the family in advocacy for the development and provision of appropriate community services for the client.</span></span></li>\n\t<li><span style=\"font-size:12pt\"><span style=\"font-size:11pt\">Maintain a comprehensive database of current community resources for staff and clients.</span></span></li>\n\t<li><span style=\"font-size:12pt\"><span style=\"font-size:11pt\">Work flexible hours to accommodate the needs of the program and
