Description POSITION SUMMARY: The Outpatient Care Navigator serves as the coordinator of services for children, their families and or adults enrolled in programs serving those with a mental illness and/or an intellectual disability. The care navigator is responsible for assessment, referrals, linking and coordination of all services, the creation of an individualized plan of treatment, and monitoring authorized services. The care navigator is the primary case-holder for the clinical record. This position is a community-based position. EDUCATION AND EXPERIENCE: Must meet the criteria as a Child Mental Health Professional (CMHP) and or a Qualified Intellectual Disability Professional (QIDP) or Qualified Mental Health Professional (QMHP). Minimum of a Bachelor’s degree in Social Work, Psychology, or a mental health-related field from an accredited school is required. Must be trained and have a minimum of 1 - 3 years of supervised experience in the examination, evaluation, and treatment of minors and their families, or adults is required. Prior experience working in a Community Mental Health setting is preferred. A current valid professional licensure/certification in the State of Michigan is preferred. Experience working in the home and/or community setting is preferred. KNOWLEDGE, SKILLS, AND ABILITIES: Knowledge of various systems operating in Wayne County (i.e. Community Mental Health, Department of Social Services, Juvenile Court, Probate Court, School). Skills in working with children, adolescents, adults and their families. Ability to elicit assistance from various systems to provide individual services to clients. Ability to negotiate alliances among various agencies to establish trust and to articulate mutual interests. Knowledge and experience using an electronic client medical record. Ability to work cooperatively with all program staff and support services personnel. Ability to advance plan and organize time effectively. Ability to manage multiple priorities, function independently and demonstrate good follow-through. Must have a working knowledge of all SFS programs. Ability to always represent SFS professionally. Ability to effectively communicate orally and in writing. Must respect the confidentiality of all program participants. Must be able to relate to all members of the community regardless of race, age, gender, ethnic origin, etc. Ability to participate in a multi-disciplinary approach to treatment and case management. Must have reliable transportation to be able to meet with clients outside of the clinic setting. Ability to maintain professional and ethical standards. Ability and desire to seek personal and professional growth and skill development. Ability to meet productivity expectations as assigned by the program supervisor. DUTIES AND RESPONSIBILITIES: Maintain a minimum of monthly face-to-face contact with the identified client regarding the client’s case and needs in the home, school, and/or other community setting. Develop/maintain liaison relationships with various community facilities. Develop, per agency policy, a written client comprehensive assessment and service plan, which identifies the needs of the client/family. Assist the family in advocacy for the development and provision of appropriate community services for the client. Maintain a comprehensive database of current community resources for staff and clients. Work flexible hours to accommodate the needs of the program and clients. Participate in and contribute to staff case reviews and staff training in collaboration with mental health therapists. Meet productivity expectations as assigned by program supervisor. Work cooperatively as a member of a team with program staff and community resources. Responsible for keeping paperwork in order and up to date in the electronic client record. Adhere to all SFS policies and procedures. Cooperate in any investigation related to personnel, licensing, accreditation or other circumstances. Responsible for working with team members, supervisor, and community contacts in a manner that is conducive to the philosophy and mission of the program and agency. Assist in finding and directing clients in case of an emergency such as a fire or tornado. Participate in agency meetings, training and other agency functions as needed or requested. Must be able to perform other duties as assigned. LICENSING AND OTHER REQUIREMENTS: Must have a reliable mode of transportation and possess a current valid State of Michigan chauffeur’s/driver’s license and automobile insurance and is insurable. This position is a community-based position.\\n\\n&nbsp;\\n\\nEDUCATION AND EXPERIENCE:\\n\\n\\n\\tMust meet the criteria as a Child Mental Health Professional (CMHP) and or a Qualified Intellectual Disability Professional (QIDP) or Qualified Mental Health Professional (QMHP).&nbsp; \\n\\tMinimum of a Bachelor&rsquo;s degree in Social Work, Psychology, or a mental health-related field from an accredited school is required.\\n\\tMust be trained and have a minimum of 1 - 3 years of supervised experience in the examination, evaluation, and treatment of minors and their families, or adults is required.\\n\\tPrior experience working in a Community Mental Health setting is preferred.\\n\\tA current valid professional licensure/certification in the State of Michigan is preferred.\\n\\tExperience working in the home and/or community setting is preferred.\\n\\n\\n&nbsp;\\n\\nKNOWLEDGE, SKILLS, AND ABILITIES:\\n\\n\\n\\tKnowledge of various systems operating in Wayne County (i.e. Community Mental Health, Department of Social Services, Juvenile Court, Probate Court, School).\\n\\tSkills in working with children, adolescents, adults and their families.\\n\\tAbility to elicit assistance from various systems to provide individual services to clients.\\n\\tAbility to negotiate alliances among various agencies to establish trust and to articulate mutual interests. \\n\\tKnowledge and experience using an electronic client medical record.\\n\\tAbility to work cooperatively with all program staff and support services personnel.\\n\\tAbility to advance plan and organize time effectively.\\n\\tAbility to manage multiple priorities, function independently and demonstrate good follow-through.\\n\\tMust have a working knowledge of all SFS programs.\\n\\tAbility to always represent SFS professionally.\\n\\tAbility to effectively communicate orally and in writing.\\n\\tMust respect the confidentiality of all program participants.\\n\\tMust be able to relate to all members of the community regardless of race, age, gender, ethnic origin, etc.\\n\\tAbility to participate in a multi-disciplinary approach to treatment and case management.\\n\\tMust have reliable transportation to be able to meet with clients outside of the clinic setting.\\n\\tAbility to maintain professional and ethical standards.\\n\\tAbility and desire to seek personal and professional growth and skill development.\\n\\tAbility to meet productivity expectations as assigned by the program supervisor.\\n\\n\\n&nbsp;\\n\\nDUTIES AND RESPONSIBILITIES:\\n\\n\\n\\tMaintain a minimum of monthly face-to-face contact with the identified client regarding the client&rsquo;s case and needs in the home, school, and/or other community setting.\\n\\tDevelop/maintain liaison relationships with various community facilities.\\n\\tDevelop, per agency policy, a written client comprehensive assessment and service plan, which identifies the needs of the client/family.\\n\\tAssist the family in advocacy for the development and provision of appropriate community services for the client.\\n\\tMaintain a comprehensive database of current community resources for staff and clients.\\n\\tWork flexible hours to accommodate the needs of the program and clients.\\n\\tParticipate in and contribute to staff case reviews and staff training in collaboration with mental health therapists.\\n\\tMeet productivity expectations as assigned by program supervisor.\\n\\tWork cooperatively as a member of a team with program staff and community resources.\\n\\tResponsible for keeping paperwork in order and up to date in the electronic client record.\\n\\tAdhere to all SFS policies and procedures.\\n\\tCooperate in any investigation related to personnel, licensing, accreditation or other circumstances.\\n\\tResponsible for working with team members, supervisor, and community contacts in a manner that is conducive to the philosophy and mission of the program and agency.\\n\\tAssist in finding and directing clients in case of an emergency such as a fire or tornado.\\n\\tParticipate in agency meetings, training and other agency functions as needed or requested.\\n\\tMust be able to perform other duties as assigned.\\n\\n\\n&nbsp;\\n\\nLICENSING AND OTHER REQUIREMENTS: \\n\\nMust have a reliable mode of transportation and possess a current valid State of Michigan chauffeur&rsquo;s/driver&rsquo;s license and automobile insurance and is insurable.
