    "requisitionDescription": "<div><link href=\"https://static.workforcenow.adp.com/mas/mdf-components/24.32.25/styles/froala_editor.pkgd.min.css\" rel=\"stylesheet\" type=\"text/css\">&nbsp;<link href=\"https://static.workforcenow.adp.com/mas/mdf-components/24.32.25/styles/froala_style.min.css\" rel=\"stylesheet\" type=\"text/css\"><div class=\"fr-view\"><p id=\"isPasted\"><strong><em>PRIMARY FUNCTIONS:&nbsp;</em></strong>The Family Nurse Practitioner provides quality clinical services to patients of all ages within a community-based setting that promotes healthy lifestyles, prevention of illness and injury and management of acute and chronic conditions. &nbsp;Activities involve direct patient care, physical assessment, diagnosis, treatment and referral for specialty services as needed, and follow-up. &nbsp;</p><p>The Family Nurse Practitioner provides care in collaboration with the center&rsquo;s Physicians and Chief Medical Officer.</p><p>Demonstrates a sincere dedication and loyalty to the philosophy and mission of Neighborhood Health.</p><p><strong>DUTIES/ RESPONSIBILITIES:</strong> (<strong><em>Essential Functions:</em></strong><em>&nbsp;Duties and responsibilities noted with bullets are considered to be the essential functions of the job).</em></p><ul><li>Provides medical care and advice to patients of all ages as indicated and according to protocols and evidence-based practice; prescribes medication and treatment according to protocol, formulary and evidence-based practice. &nbsp;</li><li>Performs patient history and physical assessment; utilizes laboratory and diagnostic tests for differential diagnoses, with follow up treatment as medically indicated.</li><li>Orders recommended screenings and preventive treatments as recommended by the Neighborhood Health Quality Assurance program and standard clinical practice.</li><li>Maintains electronic patient medical record in accordance with Neighborhood Health documentation policy.</li><li>Refers for specialty care and follows up with patient as indicated.</li><li>Collaborates with nursing, health education, behavioral health, dental, referral coordinators, pharmacy and other support staff to assure maximum patient access to support services.</li><li>Works closely with clinical staff to ensure a team approach to the delivery of services and a holistic approach to patient care.</li><li>Participates in Neighborhood Health peer review program.</li><li>Participates on Health Center committees as appointed by Chief Medical Officer</li><li>Attends regular staff meeting and trainings as required by Neighborhood Health</li></ul><p><strong>OTHER DUTIES:</strong></p><ul><li>Assist with developing, coordinating and recommending changes for the improvement of workflow in the program or section area. &nbsp;</li><li>Assists with developing and recommending changes to improve quality of patient care.&nbsp;</li><li>May be assigned to various project lead roles to advance the development of the department and support ad hoc needs.</li><li>Able to work under minimal supervision.</li><li>Employee will be asked to perform additional duties and take on other responsibilities as dictated by events and circumstances.</li><li><br></li></ul><p><strong>QUALIFICATION/EDUCATION and/or EXPERIENCE REQUIREMENTS:</strong>&nbsp;</p><ul><li>Graduate of an accredited Nurse Practitioner Program</li><li>Current Advanced Practice Registered Nurse (APRN) Licensure in Virginia</li><li>Current Nurse Practitioner certification in Adult or Family Practice</li><li>Current provider card in Basic Life Support (BLS)&nbsp;</li><li>1 or more years&rsquo; experience as a Nurse Practitioner&nbsp;</li><li>Excellent interpersonal communication skills and the ability to work effectively with a diverse workforce and client base. &nbsp; &nbsp;</li><li>Experience with information technology, electronic health records, medical records, culturally diverse populations and care of underserved populations.&nbsp;</li><li>Bilingual in English/Spanish very strongly preferred.</li></ul></div></div>\n",
