The Developmental Division strives to empower people with intellectual and developmental disabilities to live as they choose, actively engaged in the community, pursuing their goals and interests through a choice of supported residential options, health services, meaningful activities, and employment opportunities. Responsibilities The New York Foundling has served people with developmental disabilities since 1974, beginning with children and gradually expanding services to all ages, including the growing population of elders with developmental disabilities. The Developmental Division strives to empower people with intellectual and developmental disabilities to live as they choose, actively engaged in the community, pursuing their goals and interests through a choice of supported residential options, health services, meaningful activities, and employment opportunities.-Responsible for the coordination, advocacy, follow-up, and administration of residents medical care appointments, logs, and medications.-Provide direct care and support for adults residing in our group homes.-Provide services in accordance with Residential Habilitation Plans, treatment plans, as well as The New York Foundling and OPWDD guidelines to develop skills required for a more independent lifestyle.-Coordinate, transport and attend all medical care appointments for individuals residing in assigned residence(s) in a safe and timely manner.-Communicate and advocate with medical care providers, nurses, clinical, management and program staff in regards to residents medical treatment plans.-Update all medical books/documentation as necessary and communicate all changes to appropriate persons as required.-Administer medications and document books/logs accurately and in a neat and timely manner.-Develop resident's basic living skills (e.g., social, domestic, and hygiene) through instruction and encouragement.-Assist and document the development and implementation of long and short-term goals for resident, as developed in the Habilitation Plan under the supervision or direction of the Program Manager.-Develop and maintain a positive, effective and professional relationship with residents, families, staff, administration, case management, and other service providers and assist in coordinating with any and all their needs concerning residents.-Perform or assist residents with personal hygiene as needed (e.g., bathing, brushing teeth, shaving, nail clipping, dressing, bodily functions, and service recipient appearance). Personal care services that are assigned by a health professional may include observation, reporting and documentation of changes in the status of the person or in body functions. This may include reading and recording the persons temperature, pulse and respirations and other needed personal care services.-Perform or assist residents in basic domestic duties as needed (e.g., cooking, cleaning, laundry, shopping).-Maintain records and complete paperwork as required by The New York Foundling and Office for People with Developmental Disabilities (OPWDD) (e.g., timesheets, mileage, petty cash, medication logs, resident's progress, visitor logs, Incident Reports and goal data).-Adhere to resident's behavior and health management plans (e.g., administration of medication, use of behavior modification techniques, dietary restrictions).-Maintain a safe environment for the residents; prevent harm to service recipient, self, and others. Report any safety concerns to management in a timely manner.-Manage residents and agency's funds in accordance with The New York Foundling policies and procedures.-Serve as a good role model to residents.-Maintain current certification as required by The New York Foundling and the residents Individual Service Plan. (e.g., CPR, First Aid, AMAP, SCIP-R and all other required by The New York Foundling policies and procedures, or residents specific needs).-Keep up-to-date with current The New York Foundling and OPWDD policies and procedures.-Complete other duties as assigned.Salary Range: $18.87 - 20.76/hourThe New York Foundling is committed to attracting and retaining a diverse employee population, the Foundling will honor your experiences, perspectives and unique identity.
