Midland Care Connection
Midland Care Connection is a not-for-profit, community-based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.
PACE Program
The PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.
Community Resource Specialist
Helps older adults remain safely and independently in their homes by connecting them with healthcare services, financial assistance, and community resources. The role serves as a link between participants, families, healthcare providers, and local agencies to ensure seniors receive the support they need. Responsibilities often include helping participants access housing, transportation, food assistance, utility support, and public benefits such as Medicaid and Medicare. The specialist also works closely with nurses, social workers, therapists, and other members of the interdisciplinary PACE team to coordinate care and advocate for participants’ needs. In addition to resource coordination, the position involves educating families about available services, maintaining participant documentation, and providing emotional support during challenging situations. Overall, the goal of the role is to improve quality of life for elderly participants while helping them avoid unnecessary nursing home placement through comprehensive community-based support.
Qualifications
Required
High school diploma or equivalent (some positions may require an associate or bachelor’s degree in human services, social work, healthcare, or a related field)
Experience working with older adults, healthcare, social services, case management, or community outreach
Knowledge of community resources, Medicaid, Medicare, and public assistance programs
Strong communication and interpersonal skills
Ability to coordinate services and work collaboratively with an interdisciplinary healthcare team
Organizational and documentation skills for maintaining participant records
Valid driver’s license and reliable transportation may be required for community visits
Ability to maintain confidentiality and work with sensitive participant information
Preferred
Bachelor’s degree in social work, psychology, human services, gerontology, or a related field
Previous experience in a PACE program, senior services, behavioral health, or healthcare setting
Experience assisting clients with benefits applications and care coordination
Bilingual communication skills
Familiarity with electronic medical records or healthcare documentation systems
Crisis intervention or advocacy experience
Knowledge of aging-related programs and long-term care services
Strong networking skills and established connections with local community agencies
