The RN is skilled at assessment and management of pain and other complex symptoms,&nbsp;including education and support to patient and family. The mission of the home health team is to meet&nbsp;the unique needs of clients and families with services tailored to assist them in achieving optimal level of&nbsp;function.</p>\n\n<p><strong>Job Duties:</strong></p>\n\n<ul>\n\t<li>Initial and ongoing comprehensive assessment of the client&rsquo;s needs, including Outcome&nbsp;and Assessment Information Set (OASIS) assessments at appropriate time points.&nbsp;</li>\n\t<li>Initiating the plan of care and revising as necessary. The home health care plan is&nbsp;designed to the unique needs of clients and families with services tailored to assist them&nbsp;in achieving optimal level of function.&nbsp;</li>\n\t<li>Providing those services and/or treatments requiring substantial and specialized nursing&nbsp;skill.&nbsp;</li>\n\t<li>Counseling the client and family in meeting nursing and related needs.&nbsp;</li>\n\t<li>Initiating appropriate preventative and rehabilitative nursing procedures.&nbsp;</li>\n\t<li>Preparing clinical and progress notes.&nbsp;</li>\n\t<li>Coordination of services.&nbsp;</li>\n\t<li>Referral to other services as needed.&nbsp;</li>\n\t<li>Informing the physician and other staff of changes in the client&rsquo;s needs.&nbsp;</li>\n\t<li>Evaluating outcomes of care.&nbsp;</li>\n\t<li>Supervision of LPN/LVNs and paraprofessionals providing services.
