The CM will be expected to work within the policies and procedures of Family &amp; Community Services, Inc., and work toward the maintenance of good service standards and professional practices by utilizing the trauma-informed care model. Each CM will be responsible for a case load of up to 25 veteran residents into permanent housing.</p><p><br></p><p><u><strong>ESSENTIAL RESPONSIBILITIES:</strong></u></p><ul><li>Complete assessments of veteran resident and/or veteran household’s needs and to develop an appropriate support plan with the veteran, and if needed the family, with a focus on obtaining or maintaining permanent housing.</li><li>While providing services with a trauma-informed care approach, the CM will assist with provision of a wide range of practical, personal, social, and general care services for the veteran resident and/or veteran household.</li><li>Assists veteran residents and/or veteran households in developing realistic and achievable housing plans that focus on maintaining housing stability and include measurable action steps and timetables.&nbsp;</li><li>To undertake monitoring, reviewing, and recording of the agreed upon support plan according to the changing needs of the veteran resident and/or veteran household.</li><li>To promote veteran resident and/or veteran household involvement by maximizing their participation in the decision-making process within the services.</li><li>To communicate and liaise with various agencies (e.g. Department of Veterans Affairs, Veterans Services Commission, Probation Services, Housing Authority, Landlords, Department of Jobs and Family Services, Legal and Consumer Credit Organizations).</li><li>To accompany veteran resident and/or veteran household to appointments with any of the above agencies as necessary.</li><li>Maintains regular contact with the veteran resident and/or household including childcare assessments, crisis intervention, referrals, and ongoing life skills development.</li><li>Assures that veteran resident future and/or veteran household housing is habitable according to established organizational standards.</li><li>Collects, provides and updates required data for the HMIS database.&nbsp;</li><li>Supports Social Service Workers and their duties in their absence or as needed&nbsp;</li><li>To provide staff support when on-call</li><li>To assist in providing coverage for open shifts and call offs when on-call.</li><li>Travels to community locations where homeless and at-risk veterans congregate to ensure effective local outreach.</li><li>To promote anti-discriminatory practice ensuring that no resident is discriminated against on the grounds of race, class, gender, religious beliefs, disability, or sexual orientation.</li><li>To be able to adapt to the environment you are working in, this could be part of a team or own initiative in a variety of settings</li><li>To apply Housing First, Trauma Informed Care, Harm Reduction principles and to use Motivational Interviewing, and De-escalation Techniques in working with veterans of the program as appropriate.</li><li>Attend meetings, trainings, and conferences as requested.</li><li>Participates in community-wide events promoting services for veterans.</li><li>To foster positive working relationships with other staff members.</li><li>Help SSSW, all staff, and veteran’s residents maintain up-to-date knowledge of internal fire regulations and procedures and any other emergency procedures ensures health and safety standards are maintained.</li><li>Complies with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements.</li><li>To maintain all records and logs.</li><li>Other duties as assigned.</li></ul><p><br></p>",
