About Radius Community Health & Healing (formerly Boyle McCauley Health Centre) Radius Community Health & Healing provides interdisciplinary, team based, integrated, comprehensive, and person-centered primary health care for vulnerable Edmontonians with complex needs. Through primary care and health promotion, Radius Community Health & Healing improves the quality of life and well-being of populations and individuals who face multiple barriers to accessing health services. Radius achieves this by providing the highest standard of evidence-informed and client focused inter-professional care. We collaborate with our clients to address the broad determinants of health. Job Description: Licensed Practical Nurses (LPNs) play a critical role within the Radius team. Our clinical teams consist of skilled professionals, including nurses, physicians, nurse practitioners, medical office assistants, outreach workers, health advocates, mental health therapists, and other specialists, all working together to provide comprehensive, wrap-around care to Edmonton’s most marginalized populations. At Radius, LPNs are empowered to practice to their full scope within a primary health care setting, serving as leaders who demonstrate a high level of independence and collaboration. They work closely with other team members to ensure that every client receives wrap-around care, addressing their medical, social, and psychological needs with compassion and expertise. Current opportunities: Radius provides primary care to individuals that may face systemic barriers when accessing health services. Our services provided are multidisciplinary, ranging from medical care to social services. We are looking for full-time, part-time, temporary, and casual. The Programs that we are currently LPNs for are as follows: Main Clinic and Outreach Team: Licensed Practical Nurses (LPNs) at Radius are required to work to their full scope of practice within a primary health care setting. LPNs in this environment are leaders, working with a high degree of independence and in collaboration with other healthcare professionals to ensure that all social, psychological, and medical needs of clients are met. The LPNs at Radius provide a range of services, including assessment and triage, wound care, and foot care, as well as immunizations and specimen collection. They also manage medications, administering both oral and injectable medications, and are involved in overdose response and prevention. LPNs assist providers with medical procedures and play a key role in care coordination, handling tasks such as referrals, appointment bookings, and the completion of forms. Additionally, they offer health education and advocate for clients' needs. Supervised Consumption Site: Supervised consumption services are critical in saving lives and improving community health. These services provide individuals with a safe, clean environment where they can bring their own drugs and use them under the supervision of trained medical and support staff. The presence of medical professionals ensures that in the event of an overdose, immediate assistance can be provided, preventing potentially fatal outcomes. In addition to preventing overdoses, supervised consumption sites also help reduce the transmission of infectious diseases by offering clean needles and other harm reduction tools. The Licensed Practical Nurse (LPN) supervises clients in a safe, non-judgmental environment, monitoring their self-administration of substances (by injection or oral consumption) in accordance with established standards. The LPN works with clients who may have various health challenges, including substance use, communicable diseases, and chronic illnesses. Many of the clients we serve may also face stigma and marginalization. The LPN collaborates with a multi-disciplinary team to provide trauma-informed, client-centered care, focusing on health promotion, risk management, and prevention. Respite to Recovery (R2R): The Licensed Practical Nurse (LPN) for the Respite to Recovery (R2R) program at Radius Community Health and Healing works in collaboration with a partnering shelter to serve individuals experiencing homelessness who are ill but medically stable enough to temporarily convalesce in a shelter or transitional housing setting with medical support. The LPN supports individuals as they recover from acute illness or injury, facilitating connections to community social supports and ongoing primary health care services through health assessments, medical treatment, and referrals. In addition, the LPN provides education on harm reduction practices and disease prevention while promoting healthy lifestyle choices that align with each individual’s health needs and circumstances. General nursing duties include wound care and foot care, assessment and triage, immunizations, specimen collection, medication administration, overdose response and prevention, care coordination, health education, and client advocacy. The LPN also assists providers as required and works cooperatively with the partnering shelter’s staff and management, following the shelter’s guidelines and policies. Pathways: The Pathways to Housing Program in Edmonton helps end homelessness for individuals with serious mental illnesses, often coupled with addictions and other challenges. It provides permanent independent housing without requiring treatment or sobriety, along with community outreach support and treatment based on consumer choice and harm reduction principles. The LPN’s work as part of an Assertive Community Treatment (ACT) team, offering comprehensive, person-centered support, including housing assistance, life skills, and daily living tasks, in addition to specialized treatment and rehabilitation services. The team uses evidence-based practices and a team case management approach to create individualized treatment plans. STBBI: STBBIs or Sexually Transmitted and Blood-Borne Infections can affect individuals in various ways and may require medical attention, counseling, education, and support to manage prevention, treatment, and overall well-being. The STBBI nurse works within a multidisciplinary team to assess, plan, implement, and evaluate services for clients with or at risk of sexually transmitted or bloodborne infections. Responsibilities include assessments, screenings, diagnoses, treatments, and client education. The nurse will provide counseling, advocacy, and utilize critical thinking and problem-solving. The role involves working in various settings, including Radius' main clinic, outreach locations, and community spaces such as drop-in centers and shelters. Navigation Centre: This role will be based primarily offsite at the Navigation Centre. The purpose of the Navigation Centre is to connect people living in encampments with a number of crucial services in one location. This role will be based primarily offsite at the Navigation Centre. Radius LPNs are essential members of the team-based care model at Radius, working alongside nurses, physicians, nurse practitioners, medical office assistants, outreach workers, health advocates, and mental health therapists to provide comprehensive, wrap-around care to Edmonton’s most marginalized populations. LPNs work to the full scope of their practice within a primary healthcare setting, addressing all aspects of client needs. As leaders and independent practitioners, they collaborate with the team to ensure high-quality care, continuity of services, and access to both medical and social supports for clients. Key Responsibilities: • Work collaboratively within a multidisciplinary team to provide healthcare services in various clinical and/or community-based settings. • Provide direct nursing care including but not limited to assessments, triage, wound care, foot care, immunizations, specimen collection, medication administration (oral and injectable medications) • Overdose prevention and response • Manage patient appointments and maintain accurate medical records using Electronic Medical Records (EMR). • Provide client education. • Engage with clients to foster relationships, providing advocacy and facilitating connections to social and medical supports. • Client Advocacy Qualifications: • Active registration with the CLHA (College of Alberta Licensed practical Nurses and Health care Aids) including disclosure of any practice restrictions. • Experience with marginalized populations, including those experiencing homelessness, addiction, and mental health challenges, is an asset. • Knowledge of and experience working within a trauma-informed, harm-reduction model of care is an asset. • Ability to balance multiple priorities and work under pressure with strong problem-solving skills. • Non-Violent Crisis Intervention training, Mental Health First Aid, and Applied Suicide Intervention Training are considered assets. • Knowledge of community resources in Edmonton, such as social support services. Skills and Competencies: • Able to balance numerous demands and conflicting priorities, strong problem-solving skills. • Strong computer proficiency with Microsoft 365 suite, experience with MedAccess EMR an asset. • Excellent verbal and written communication skills. • Must be able to operate with a trauma-informed approach. • Excellent understanding of cultural awareness is an asset. • Understanding and ability to apply principles of Harm Reduction Additional Requirements: • Police Information Check Including the Vulnerable Sector Check - Must be current and must be renewed every 3 years. • Intervention Record Check - Must be current and must be renewed every 3 years. • Immunization Record & Mantoux Testing • CPR Registration - Must be currently active. • Registration with CLHA - Including disclosure of any practice restrictions.
