Brighton Ave</em>) maintains quality clinical care for residents/clients through direct supervision of nursing staff. Communicates modifications to policy and procedures and oversees compliance, serving as a resource to clinical staff. Manages the daily activities of the clinical staff.</p>\n<p><strong>Shift:</strong> Part Time & Full Time Evenings 3:00pm – 11:30pm </p>\n<p><strong>Pay:</strong> $44.47 - $51.83 per hour</p>\n<p>Plus $7.50 Per Hour - Evening Shift Differential Offered!</p>\n<h2>Responsibilities</h2>\n<p><strong>Job Duties specific to this position:</strong></p>\n<p> </p>\n<p><strong>Resident Care</strong></p>\n<ul>\n <li>Supervises the clinical care planning and delivery processes to ensure the coordination of quality patient care. Reviews and updates the care plan accordingly.</li>\n <li>Assesses residents/clients upon admission and periodically as conditions change.</li>\n <li>Performs rounds to evaluate patient condition.</li>\n <li>Provides direct patient care when indicated.</li>\n <li>Obtains and implements Physician Orders, checking for accuracy and completeness.</li>\n <li>Communicates effectively with the client/resident, family and interdisciplinary team and ensures staff members communicate effectively as well. Delivers education and instruction to residents, clients, families and caregivers when needed.</li>\n <li>Communicates between shifts regarding changes in resident/client conditions.</li>\n <li>Ensures a safe and secure environment for clients/residents, staff and visitors.</li>\n <li>Collaborates with other members of the interdisciplinary team.</li>\n <li>Maintains the highest level of customer service within the area of responsibility.</li>\n <li>Provides clinical assessment to MD/NPs on call for quality outcomes delivered to patients.</li>\n</ul>\n<p><strong>Management and Employee Development:</strong></p>\n<ul>\n <li>Provides daily leadership, direction and prioritization to personnel in accordance with organizational/ILS/RHCF goals and mission.</li>\n <li>Plans, implements and evaluates the clinical competencies and on-going educational/orientation needs of the staff. Updates orientation or training materials as needed. Shares current knowledge and up-to-date clinical expertise with staff through formal and informal education. Takes responsibility for own education and stays clinically current.</li>\n</ul>\n<p><strong>Budget:</strong></p>\n<ul>\n <li>Develops and monitors the department staffing pattern to meet the needs of the patients, within budget parameters.</li>\n <li>Checks staff overtime and shifts/hours worked.</li>\n</ul>\n<p><strong>Compliance:</strong></p>\n<ul>\n <li>Enforces the bargaining unit contract fairly and consistently. Addresses issues in a timely and appropriate manner.</li>\n <li>Ensures all re-orientation requirements for nursing staff are met.</li>\n <li>Ensures all regulations, policies and procedures are adhered to by nursing staff and self.</li>\n <li>Monitors and maintains compliance with third party agencies.</li>\n</ul>\n<p><strong>Documentation:</strong></p>\n<ul>\n <li>Directs the processes that facilitate the thorough and accurate completion of required documentation. Examples include but are not limited to: skin//wound reports, ADLs, weights, nourishment, monthly MAR/TAR checks, Admissions, Discharges, incident reports, investigations, audits, PRIs.</li>\n</ul>\n<p><strong>Quality:</strong></p>\n<ul>\n <li>Monitors adherence to the patient care standards through QAPI/QA processes, taking remedial action when needed.</li>\n <li>Participates in Quality Assurance Initiatives. Collects and provides data as required.</li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li>At least 3 years’ experience in nursing preferably with 1-2 years in an acute/long term care setting.</li>\n <li>Degree from an accredited school of nursing. BSN or BS in related field preferred.</li>\n <li>Current New York State RN license and registration in good standing required.</li>\n <li>Proven patient assessment skills.</li>\n <li>Adheres to best practice nursing care.</li>\n <li>Ability to supervise and guide staff while carrying out the duties of an RN.</li>\n <li>Proficient in the care planning process.</li>\n <li>Outstanding communication skills and the ability to communicate effectively with clients, residents, and families from diverse backgrounds who may have physical, sensory and mental impairments.</li>\n <li>Ability to manage multiple priorities in a busy environment.</li>\n <li>Ability and knowledge to respond in emergency situations.</li>\n <li>Ability to analyze problems, plan resolution strategies, implement solutions and evaluate outcomes.</li>\n <li>Proficient in computer programs and able to learn new software rapidly</li>\n</ul>\n<p><em>An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements</em>.</p>\n<p><strong>Physical Requirements </strong></p>\n<p>Walking/Standing 50.00%</p>\n<p>Sitting 40.00%</p>\n<p>Lifting 10.00% 50 lbs</p>\n<p><em>The above statements are intended to describe the general nature and level of work being performed.
