Transform Advanced Illness Care.</strong></p>\n<p> </p>\n<p>We are seeking a dedicated Nurse Practitioner to join our Advanced Illness Management (AIM) team. In this role, you will provide expert diagnosis, treatment, consultation, and follow-up for patients with complex needs, working collaboratively with the Professional Services Corporation (PSC) Physician, Primary Care Physicians, and interdisciplinary palliative care teams.</p>\n<p> </p>\n<p><strong>Key Responsibilities:</strong></p>\n<ul>\n <li><p>Collaborate with PSC Physician, Primary Care Physician, and palliative care team members to deliver comprehensive advanced illness management and palliative care.</p></li>\n <li><p>Take full responsibility for patient care from initial assessment through discharge, addressing disease-specific changes, functional decline, symptom management, nutrition, cognition, and caregiver support.</p></li>\n <li><p>Develop individualized care plans in partnership with patients, families, and care teams.</p></li>\n <li><p>Facilitate access to treatments, services, and durable medical equipment as needed per care plan.</p></li>\n <li><p>Participate in evening and weekend call rotations providing clinically competent on-call services.</p></li>\n <li><p>Prescribe medications including controlled substances within licensure and state law.</p></li>\n <li><p>Order and interpret lab tests, imaging, and other diagnostics relevant to patient care.</p></li>\n <li><p>Maintain bi-weekly communication with the PSC Physician and update care plans based on ongoing assessment.</p></li>\n <li><p>Collaborate with social workers, RN care managers, and other supportive care providers to optimize patient outcomes.</p></li>\n <li><p>Ensure all clinical documentation meets standards to support medical practice and billing compliance.</p></li>\n <li><p>Monitor and report on palliative medicine quality measures and documentation requirements.</p></li>\n</ul>\n<h2>About You</h2>\n<p><strong>Specialized Knowledge and Skills:</strong></p>\n<ul>\n <li><p>Strong leadership and interpersonal communication skills.</p></li>\n <li><p>Independent decision-making ability and self-motivation.</p></li>\n <li><p>In-depth knowledge of palliative medicine principles, Medicare/Medicaid regulations, value-based care, and interdisciplinary processes.</p></li>\n <li><p>Meets continuing education requirements set by Gentiva and licensing boards.</p></li>\n <li><p>Proficiency with electronic medical record (EMR) software.</p></li>\n <li><p>Willingness to travel 10-20% for training and education.</p></li>\n</ul>\n<p><strong>Education and Experience:</strong></p>\n<ul>\n <li><p>Master of Science in Nursing (MSN) or equivalent from an accredited institution.</p></li>\n <li><p>Minimum two years of Nurse Practitioner clinical experience.</p></li>\n <li><p>Specialty in Family or Adult Medicine preferred.</p></li>\n <li><p>Experience with value-based care models and Part B billing preferred.</p></li>\n <li><p>Ability to work autonomously in an advanced practice role.</p></li>\n</ul>\n<p><strong>Licenses, Certifications, and Agreements:</strong></p>\n<ul>\n <li><p>Current RN license or equivalent as required by state law.</p></li>\n <li>Active and Unrestricted Nurse Practitioner certification/license without board actions</li>\n <li><p>Collaborative agreement with supervising physician from the Professional Services Corporation.</p></li>\n <li><p>Part B Provider certification.</p></li>\n <li><p>Federal DEA license;
