You’ll be responsible for increasing brand awareness, generating client referrals, and fostering strategic partnerships within local communities. Your ability to connect with diverse groups will help ensure True Care’s services are accessible, trusted, and visible across the region.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Key Responsibilities:</span></p>\n<ul>\n<li>Proactively identify and cultivate referral sources to grow True Care’s client base.</li>\n<li>Build and maintain relationships with community organizations, healthcare providers, religious institutions, and local groups.</li>\n<li>Nurture long-term partnerships that align with True Care’s mission and support sustainable community outreach.</li>\n<li>Design and implement outreach strategies tailored to local community needs, including developing culturally relevant materials and campaigns.</li>\n<li>Represent True Care at community events, health fairs, and networking opportunities; serve as a brand ambassador to promote our services.</li>\n<li>Maintain accurate and timely documentation of outreach activities and leads in Salesforce.</li>\n<li>Work closely with internal teams to ensure seamless follow-up, intake, and onboarding processes for referred clients.</li>\n<li>Adapt to organizational needs and support special projects as assigned.<br><br></li>\n</ul>\n<p><span style=\"font-weight: bold\">Key Performance Indicators (KPIs):</span></p>\n<ul>\n<li><span style=\"font-weight: bold\">Referral Growth:</span> Generate 15–20 new client referrals per month.</li>\n<li><span style=\"font-weight: bold\">Community Engagement:</span> Conduct outreach to at least 4 new community members or organizations per month.</li>\n<li><span style=\"font-weight: bold\">Strategic Partnerships:</span> Establish 3+ new partnerships per quarter.</li>\n<li><span style=\"font-weight: bold\">CRM Accuracy:</span> Ensure 100% accurate data entry in Salesforce, updated within 48 hours of contact.</li>\n<li><span style=\"font-weight: bold\">Start-of-Care Conversion:</span> Achieve 7+ successful service starts per month.<br><br></li>\n</ul>\n<p><span style=\"font-weight: bold\">Qualifications:</span></p>\n<ul>\n<li>3+ years of experience in community outreach, business development, account management, or sales — ideally within the healthcare, nonprofit, or service-based sectors</li>\n<li>Proven ability to generate leads, build partnerships, and convert prospects into clients</li>\n<li>Strong communication, public speaking, and relationship-building skills</li>\n<li>Cultural competency and a demonstrated ability to engage with diverse communities</li>\n<li>Proficiency in Microsoft Office and Salesforce CRM (or similar CRM tools)</li>\n<li>Valid driver’s license and reliable transportation to travel locally for outreach activities<br></li>\n<li>Bilingual Spanish required</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Benefits:</span><span style=\"color: rgb(34, 34, 34)\"><br></span></p>\n<ul>\n<li>Health, dental, and vision insurance</li>\n<li>401(k) with company match</li>\n<li>Paid Vacation, Company Holidays, Sick Leave, and Floating Holidays<br></li>\n<li>Professional development opportunities</li>\n<li>Mission-driven work in an innovative healthcare space</li>\n</ul>",
