          "text": "<p><strong>AHRC NYC is looking to fill two Part-Time Licensed Practical Nurses (LPN) positions at their ICF residence in Manhattan. The LPN will monitor health and safety needs of people supported living in our residences to promote wellness.</strong></p><p><strong><u>Positions available - Flexible schedules</u></strong></p><p><strong>1. 24 hours per week</strong></p><p><strong>2. 36 hours per week</strong></p><p><strong><u>Location:</u> 126 East 126th Street in Manhattan.</strong></p><p><strong><u>Pay Rate</u>: $35-$36,50 per hour</strong></p><p><strong><u>ESSENTIAL RESPONSIBILITIES (i</u>nclude but are not limited to):</strong></p><ul><li>Supervise and participate with the direct support staff tasks related to personal hygiene, Activities of Daily Living, incontinence care, and environmental cleanliness, ensuring these tasks are properly performed.</li><li>Supervise and participate with the direct support staff in mealtime and feeding protocols, to assure adherence to recommended nutritional and medical guidelines.</li><li>Perform medication administration as per procedure outline.</li><li>Report and oversee infection control practices as outlined per agency and per program- specific exposure control plans.</li><li>Oversee and ensure adequate basic First Aid supplies are available as necessary for the safety of people supported, per departmental guidelines.</li><li>Report and document medical findings for RN review and for inclusion to a person’s plan of nursing services.</li><li>Reporting immediately any significant deviations in the medical status of a person supported to RN Supervisor.</li><li>Communicate, report and document the issues and concerns of the person supported to primary medical provider and RN Supervisor.</li><li>Communicate with families and maintain family contacts to inform and discuss pertinent medical concerns of people supported.</li><li>Provide post-operative, routine or preventative medical treatments and interventions as per MD or RN order (i.e., G-tube feedings, wound and skin care, urinary drainage systems maintenance, blood glucose monitoring, passive and active range of motion therapy).</li><li>Obtain baseline and monitor medical information (Vital Sign Monitoring, Weights).</li><li>Ensure ongoing compliance and quality assurance with Federal and State regulations pertinent to the operation of the residence.</li><li>Utilize appropriate and expected medical charting skills.</li><li>Attend medical appointments as requested by RN Supervisor as individual LPN scheduling permits.</li><li>Observe and follow through on established emergency notification procedures in a manner which will ensure the safety and welfare of all people supported.</li><li>·Provide coverage in accordance with established staffing schedules.</li><li>·Attend and successfully complete all necessary training programs.</li><li>Perform other duties as assigned</li></ul>",
