In addition to their case worker, clients will have access to the After Hours Support Specialist for assistance with any additional needs that include essential life skills and crisis intervention.</p>\n<p> </p>\n<p>    Provide professional, skilled and consistent after hours support services to active Supportive Housing clients including but not limited to, the coordination and delivery of services related to crisis intervention, grocery shopping, transportation to appointments, housekeeping, and basic daily living skills. Work in conjunction with the Supportive Housing Caseworkers to ensure consistency of service delivery between after-hours support, and regular business hours, and to maintain housing and all individualized housing goals. Research and refer clients to community resources and other organizations, and network with community service providers. Complete all necessary reporting forms for government agencies and funding sources (UCR’s, progress notes, referrals, and statistics). Maintain client case files. Maintain appropriate communication with caseworkers, supervisors and other funding sources, notifying appropriate parties of any safety issues in a timely manner. Maintain guidelines established by the funding sources and Catholic Charities policies and procedures. Perform additional duties related to housing individuals and helping them remain housed, including but not limited to furnishing and supplying apartments.</p>\n<p> </p>\n<p><strong>This Position is Full time at 35 hours a week (Weekend hours are 8a-4p and weekday hours range from 3p-11pm). Pay For This Position is $19.00 Per Hour.</strong></p>\n<h2>Qualifications</h2>\n<p><strong>Education and/or Experience Required at Entry:</strong></p>\n<ul>\n <li><p>Require a two-year degree, business school or trade school degree or certification in a specialized field with up to three years of relevant experience, or combination of education and experience.</p></li>\n <li><p> Relevant experience working with individuals with substance abuse/ mental health concerns</p></li>\n <li>Experience with crisis intervention and supports preferred; </li>\n <li>candidates with lived experience are preferred, and encouraged to apply.</li>\n</ul>\n<p><strong>Licenses/Certificate/Clearances:</strong> </p>\n<ul>\n <li>Valid N.Y.S. Drivers License, </li>\n <li>N.Y.S. Child Abuse Registry clearance.</li>\n</ul>\n<p><strong>Skills/Abilities</strong>: </p>\n<ul>\n <li>Basic computer skills. </li>\n <li>Creative problem solving. </li>\n <li>Negotiation skills.</li>\n</ul>\n<p><strong>Working Conditions and Environment/Physical Demands:</strong> </p>\n<ul>\n <li>Mobility sufficient to drive to and participate in meetings at client’s homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals.</li>\n</ul>\n<p>Catholic Charities of Onondaga County is dedicated to caring for those in need while promoting human development, collaboration, and the elimination of poverty and injustice; helping people in need regardless of their religion, race, ethnicity, or nationality.
