This position will specifically be covering our Long Term Care Center and our Transitional Care Unit. Specific duties include:</p><ul><li>Provide evaluation and treatment of various diagnoses/disorders in accordance with physician orders, working directly with clients and caregivers to offer assessment and instruction.&nbsp;</li><li>Develop individualized client care plans conduct client evaluations, and assess the best course of action on a case-by-case basis.&nbsp;</li><li>Instruct clients in the use of adaptive equipment as well as collaborate with the IDT (interdisciplinary team) to ensure adequate and appropriate flow of services.&nbsp;</li><li>Maintain accurate records and documentation to ensure compliance with regulatory and organization requirements, completing discharge documentation as directed.
