Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency. Serve as a public awareness representative and will be responsible foreducation related to provided services.</p>\n<h2>Qualifications</h2>\n<p>Education and experience, essential</p>\n<ul>\n <li>Must be a graduate of an approved school of clinical education</li>\n <li>Must be licensed in the state where they currently practice.</li>\n <li>Must have at least one year experience in home care.</li>\n <li>Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of a hospice.</li>\n <li>Must have basic demonstrated technology skills, including operation of a mobile device.</li>\n</ul>\n<p>Education and experience, preferred</p>\n<ul>\n <li>Demonstrated experience with and knowledge of hospice is preferred.</li>\n</ul>\n<p>Requirements</p>\n<ul>\n <li>Must possess a valid state driver license</li>\n <li>Must maintain automobile liability insurance as required by law</li>\n <li>Must maintain dependable transportation in good working condition</li>\n <li>Must be able to safely drive an automobile in all types of weather conditions</li>\n</ul>\n<h2>Additional Information</h2>\n<p><em>Enhabit Home Health & Hospice is an equal opportunity employer.
