font-size: 16px\">As a Physical Therapist, you are responsible for initial assessment, reassessment, developing care plans and setting measurable goals for clients.  You are charged with carrying out clients care as established in the Agency’s policies and procedures.</span></p>\n<p> </p>\n<p><span style=\"font-weight: bold\"><span style=\"font-family: Arial, sans-serif; font-size: 16px\">Responsibilities:</span></span></p>\n<ul>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 16px\">Perform initial assessment and ongoing evaluation and treatment</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 16px\">Develop and implement an individualized plan of care with measurable goals for patients</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 16px\">Monitor patients’ response to treatment</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 16px\">Conduct individual patient therapy regimens</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 16px\">Supervise Physical Therapy Assistants</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 16px\">Work to identify and solve clients’ concerns</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 16px\">Complete appropriate documentation according to agency’s policies and procedures</span></li>\n</ul>\n<ul>\n<li><br><span style=\"font-family: Arial, sans-serif; font-size: 16px\">Teaches, supervises, and counsels the client and family in the physical therapy program.</span></li>\n<li><br><span style=\"font-family: Arial, sans-serif; font-size: 16px\">Investigates and resolves patient/family member concerns regarding patient care.</span></li>\n<li><br><span style=\"font-family: Arial, sans-serif; font-size: 16px\">Ensures the highest quality of care is provided to patients by reviewing documentations.</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\"><span style=\"font-family: Arial, sans-serif; font-size: 16px\">Requirements:</span></span></p>\n<ul>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 16px\">Must </span><span style=\"font-family: Arial, sans-serif; font-size: 16px\">be a graduate of an accredited physical therapy program</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 16px\">Must be a licensed Physical Therapist in the State of North Carolina</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 16px\">Possess Basic Life Support (BLS) certification </span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 16px\">Knowledge of state, federal and professional laws and regulations pertaining to physical therapy in home care</span></li>\n</ul>\n<ul>\n<li><br><span style=\"font-family: Arial, sans-serif;
