Additionally, our Community Health Initiatives focus on addressing health disparities, making care more accessible to underserved populations.</span><br></p>\n<p><br></p>\n<p><span style=\"font-size: 12pt\">At POD Health, we strive to make healthcare personal, proactive, and accessible for all.</span></p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Work Schedule: Monday-Friday 9:00 AM - 5:00 PM</span><br></p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Location: Onsite | Brooklyn, NY 11218</span></p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Key Responsibilities:</span></p>\n<ul>\n<li>Collaborate with Clinical and Operational management to lead team of Medical Assistants</li>\n<li>Assist with patient and medical assistant team oversight of Chronic Care Management (CCM), Physical and Rehabilitation Medicine (PRM), Behavioral Health Integration (BHI), and other POD programs</li>\n<li>Assist with oversight of Medical Assistant documentation, workflow and metrics</li>\n<li>Monitor and review patient vital signs, alerts and other concerns</li>\n<li>Answers phones, respond to inquiries and triage calls appropriately</li>\n<li>Assist in the development and implementation of individualized care plans</li>\n<li>Respond to urgent care needs when necessary</li>\n<li>Assist with administrative tasks </li>\n<li>Screen patients and make referrals accordingly to other clinical lines</li>\n<li>Timely completion of clinical requests and follow ups</li>\n<li>Documentation audit reviews</li>\n<li>Maintain comprehensive medical records of all patient interactions</li>\n<li>Perform other related duties as needed</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Required Qualifications:</span></p>\n<ul>\n<li><span style=\"font-weight: bold\">Mandarin Chinese fluency is required.</span></li>\n<li>Graduate from an accredited Licensed Practical Nursing program.</li>\n<li>High School Diploma or equivalent GED.</li>\n<li><span style=\"font-weight: bold\">Current Licensure and registration as RN or LPN in New York State.</span></li>\n<li>Excellent communication, interpersonal, organizational and time management skills.</li>\n<li>Ability to work independently and collaboratively within a team environment.</li>\n<li>Competency with EMR Systems and Documentation.</li>\n<li>Compassionate, patient- focused, and committed to providing high-quality care.</li>\n<li>Knowledge of complying with all state requirements and facility policies and procedures.</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Preferred Qualifications:</span></p>\n<ul>\n<li>Previous experience with supervision/leading Medical Assistants.</li>\n<li>Current Licensure and registration as a RN or LPN in Florida State.</li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Benefits:</span></p>\n<ul>\n<li>Health, dental, and vision insurance</li>\n<li>401(k) with company match</li>\n<li>Paid Vacation, Company Holidays, Sick Leave, and Floating Holidays<br></li>\n<li>Professional development opportunities</li>\n<li>Mission-driven work in an innovative healthcare space</li>\n</ul>",
