    "description_html": "<p>&nbsp;</p><p><u><strong>MAJOR RESPONSIBILITIES</strong></u></p><p><br></p><p>The employee will be directly responsible for managing a caseload of individuals with chronic physical health, mental/behavioral health, and social needs.&nbsp;</p><p>Coordinate comprehensive medical and behavioral health care for patients with chronic conditions through care coordination and integration that assures access to appropriate services and improves health outcomes. The primary goal of the employee will be to assist individuals in reducing avoidable emergency room visits and hospital admissions through the provision of care management services.&nbsp;</p><p><br></p><p><u><strong>DETAILED RESPONSIBILITIES</strong></u></p><p><br></p><p>Services will be provided to the individual through linkage and referral to appropriate community services.</p><ul><li>Coordinate services with community providers, hospitals, and family members.</li><li>Complete comprehensive assessments, develop with the client care plan goals</li><li>Monitor and document services provided and progression of goals</li><li>Capture engagement of clients in detailed progress notes.</li><li>Make referrals to community and social support services.</li><li>Offering prevention and wellness support services</li></ul><p><u><strong>QUALIFICATIONS:</strong></u></p><ul><li>Experience working with individuals living with a severe mental illness</li><li>Knowledge of mental health terminology and community resources is a plus</li><li>Experience in managing a full caseload</li><li>Experience in managing time and productivity effectively and adhere to strict deadlines.</li><li>Ability to work in a fast pace environment</li><li>Excellent communication and written skills</li><li>Computer knowledge, including but not limited to Windows, Microsoft Office (Word, Excel, outlook, outlook calendar)</li><li>Ability to work as part of a team with co-workers and with other colleagues as necessary.</li><li>Experience working with a High level of comfort working with multiple electronic systems with ease.</li><li>Cultural sensitivity and the ability to relate/work with diverse groups, community agencies, schools, and the general public.</li><li>Strong organizational skills with a focus on detail.</li><li>Ability to follow directions and communicate well (orally and in writing)</li><li>Assume other duties as assigned by the Director or Supervisor of HH to complete tasks for the program.</li></ul><p><u><strong>EDUCATION/TRAINING/EXPERIENCE&nbsp;</strong></u></p><ul><li>Bachelor's degree in one of the fields listed below:</li></ul><p>Social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech, and hearing or other human services field; and</p><ul><li>One year experience in direct services to mentally disabled, chronically ill, or other Standard of Medical Under-Service.&nbsp;</li></ul><p><u>OR</u></p><ul><li>Valid Credentialed Alcoholism and Substance Abuse Counselor (CASAC) certification with two years of experience in providing direct services to people with Serious Mental Illness, developmental disabilities, or alcoholism or substance abuse; or linking individuals with Serious Mental Illness, developmental disabilities, or alcoholism, or substance abuse to a broad range of services essential to successful living in a community setting (e.g., medical, psychiatric, social, educational, legal, housing, and financial services).</li><li>A master's degree in one of the listed education fields may be substituted for one year of experience.</li></ul>",
