This role requires an individual with excellent communication and interpersonal skills, the ability to manage multiple tasks and priorities, and a strong understanding of nursing facility operations.</span></p>\n<p><br></p>\n<p><span style=\"font-weight: bold\"><span style=\"font-family: Arial, sans-serif; font-size: 15px; text-decoration: underline\">Key Responsibilities:</span></span></p>\n<ul>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 15px\">Manage an active caseload which involves monitoring and evaluating options and services to meet an individual's health needs.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 15px\">Develop and modify care plans at least annually or on an as needed basis in collaboration with the resident, their family, and facility staff.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 15px\">Ensure that nursing facility residents receive appropriate healthcare services and that their medical needs are being met.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 15px\">Notify the member of their right to choose any willing and qualified provider to provide a service on the members service plan.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 15px\">Serve as a liaison between nursing facility residents, their families, and healthcare providers.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 15px\">Coordinate and participate in interdisciplinary care conferences with nursing facility staff and healthcare providers as needed.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 15px\">Ensure compliance with all legal and regulatory requirements related to nursing facility services.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 15px\">Required to complete forty (40) hours orientation training and ongoing twenty (20) hour annual training.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 15px\">Responsible for following Amcord Care Inc. policies and procedures for document maintenance, confidentiality or records and employees' rights.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 15px\">Willingness and ability to work in the field while also having the ability to work independently with integrity in a virtual setting.</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-weight: bold\"><span style=\"font-family: Arial, sans-serif; font-size: 15px; text-decoration: underline\">Qualifications:</span></span></p>\n<ul>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 15px\">Must have a Bachelor’s degree in social work, psychology, or other related fields with practicum experience, or in lieu of a Bachelor’s degree have at least three (3) or more years’ experience in a social service or healthcare related setting. </span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 15px\">Excellent communication and interpersonal skills.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 15px\">Ability to work collaboratively with staff, program participants, and community organizations.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 15px\">Strong organizational and problem-solving skills.</span><br></li>\n</ul>\n<p><br><br></p>\n<p><span style=\"font-family: Arial, sans-serif; font-size: 15px\">*Must be willing and able to travel regularly through these areas and surrounding counties* </span></p>\n<p><br><br></p>\n<p>Pottsville, PA 17901</p>\n<p>Bethlehem, PA 18017</p>\n<p>Allentown, PA 18103-18104</p>\n<p>Shenandoah, PA 17976<br></p>\n<p>Easton, PA 18042</p>\n<p>Reading, PA 19611</p>",
