Job Summary
Serves as the primary liaison between mental health treatment facilities, MNPS schools, students, and families to ensure continuity of care and successful school reintegration for students receiving inpatient or intensive mental health services. Coordinates communication and service alignment across stakeholders to support smooth transitions from treatment settings back to the school environment. Through proactive coordination, clear communication, and ongoing monitoring, this role helps ensure that students receive consistent, integrated support that promotes stability, academic continuity, and overall well-being upon their return to school.
What you will do
Leads and facilitates the Reintegration Support process in close collaboration with the STES Teacher Liaison Specialist, School-Based Mental Health Providers (SBMHPs), and clinical partners at treatment facilities.
Ensures that individualized aftercare plans are developed, communicated, and implemented effectively, with a focus on minimizing disruption to the student’s educational experience.
Track MNPS students admitted to short-term and long-term mental health facilities and maintain timely communication with involved partners.
Attend hospital discharge meetings as needed, with parent or guardian consent, to support transition planning.
Work closely with mental health facility staff to create individualized after-care plans that facilitate smooth reintegration into the MNPS educational environment. Provide the after-care plan to the School-Based Mental Health Provider in a timely manner and assist with any required school safety planning.
Follow up with schools to ensure completion of the Reintegration Support Plan on the student’s first day back or within three days of return and that ongoing supportive services are in place.
Offer training, guidance, and ongoing support to school teams regarding the Reintegration Support Process.
Deliver comprehensive case management services to students and families post-discharge, ensuring strong connections to school supports and external community resources.
Additional Requirements
Collaborate with the STES Teacher Liaison to obtain necessary Release of Information documentation between MNPS and the mental health facility upon a student's admission.
