Definition : The BHN Nurse Care Manager Level I is a Licensed Practical Nurse (LPN) who functions as a core member of a collaborative care team including a Clinical Team Leader, Clinicians, Care Coordinators, Psychiatric Nurse Practitioner, Peer Support Specialist, healthcare consultants, and primary care partners. This role primarily supports psychiatric providers and Opioid Health Home programming under MaineCare Section 93, with a focus on medication-assisted treatment (MAT), care coordination, and direct nursing services. Experience with injections such as Sublocade and Brixadi is required. Personal Qualifications: Demonstrates maturity, initiative, sound judgment, and creativity. Strong communication skills (oral and written). Ability to work within a multidisciplinary team providing person-centered care. Valid driver’s license with clean driving record. Physical Requirements: Requires the ability to sit, stand, and walk for extended periods; perform light lifting (up to 25 pounds); use a computer for documentation; and respond appropriately to patient needs, including occasional crisis situations in a behavioral health setting.
