Care Coordination is provided to children from admission until final discharge. Care Coordination also works very closely with other departments such as Health Homes, Care Management and Case Planning to ensure that all needs are met and provided seamlessly.&nbsp;</p><p><br></p><p><strong><span style=\"font-size: 16px;\"><u>Field Placement</u></span></strong></p><p><strong>Project Assignments:</strong></p><ul><li>The social work intern will carry a caseload of 25-35 (internal interns3-5 cases)&nbsp;</li><li>will be responsible for ensuring that all medical needs are met as well as dental and behavioral health.&nbsp;</li><li>The SW intern will connect with other Care Coordinator on the team when needed as well as other team members from other programs to ensure that all services are delivered to the child/ children.</li><li>&nbsp;They will document all services into our medical health records and will assist with assigned administrative duties that may come up.</li><li>&nbsp;They will also be responsible to coordinating complex trauma assessments.</li></ul><p><br></p><p><strong>Intern will gain knowledge in the following areas:</strong></p><ul><li>Will fully understand the role as a care coordinator at a foster care agency.&nbsp;</li><li>Will also understand the meaning of team work and how to effectively document all services provided to the children in GW&rsquo;s care.&nbsp;</li><li>The intern will understand how to provide referrals and ongoing support to the children on caseload.&nbsp;</li><li>Understand the intricacies of foster care and the requirements provided by outside governing agencies such as ACS and OCFS.&nbsp;</li></ul><p>&nbsp;</p><p>&nbsp; &nbsp;</p><p>&nbsp;</p><div><div class=\"fr-view\"><div><div><div><div><div><div><div><div><p><br></p></div></div></div></div></div></div></div></div></div></div>\n    </div>\n  </div></div>\n",
