Join us on this meaningful journey where your skills, compassion and dedication will make a remarkable difference in the lives of those we serve.&nbsp;</p><p style=\"line-height: normal; margin-bottom: 0in; margin-top: 0in;\"><strong>More about our team</strong></p><p style=\"line-height: normal; margin-bottom: 0in; margin-top: 0in;\">This team works in our 12-bed inpatient Behavioral Health Unit and primarily works with an adult patient population.&nbsp;</p><p style=\"line-height: normal; margin-bottom: 0in; margin-top: 0in;\"><strong>How you’ll contribute</strong></p><p style=\"line-height: normal; margin-bottom: 8pt;\">A Social Worker who excels in this role investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems. Additional responsibilities include:&nbsp;</p><ul style=\"list-style-type: disc;\"><li><p style=\"line-height: normal; margin-bottom: 0in;\">Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.</p></li><li><p style=\"line-height: normal; margin-bottom: 0in;\">Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.</p></li><li><p style=\"line-height: normal; margin-bottom: 0in;\">Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.&nbsp;</p></li><li><p style=\"line-height: normal; margin-bottom: 0in;\">Assists with discharge planning and processes.
