Description

The Care Coordinator is responsible for providing comprehensive care coordination, funding management, resource navigation, marketing, and supportive case management services to youth and young adults participating in the Pathways to Success Program. This hybrid position combines direct participant support, program administration, funding oversight, community outreach, and stakeholder engagement to ensure participants receive the resources and services necessary to achieve their educational, vocational, housing, and personal goals. The Care Coordinator serves as the primary point of contact for participants and is responsible for assessing needs, developing individualized support plans, coordinating services, processing funding requests, monitoring expenditures, and tracking participant outcomes. The position requires strong organizational skills, attention to detail, excellent communication abilities, and a commitment to helping participants overcome barriers to success. The Care Coordinator works collaboratively with educational institutions, employers, landlords, service providers, community partners, and internal stakeholders to ensure participants receive coordinated and effective support. This role requires the ability to work independently, manage multiple priorities, maintain accurate documentation, and exercise sound judgment in a fast-paced environment. The Care Coordinator must be flexible, dependable, and able to establish positive working relationships with diverse populations. The position includes both office-based and community-based work and may require occasional evening or weekend availability to meet participant needs. Role & Responsibilities This position provides care coordination, funding oversight, and supportive services to program participants. Primary duties include but are not limited to the following: • Understand the minimum standards, policies, and requirements of the Pathways to Success Program. • Maintain an active caseload of program participants. • Contact new referrals within established program timeframes. • Complete intake assessments and eligibility screenings for participants seeking assistance. • Assess participants' educational, vocational, housing, transportation, healthcare, and supportive service needs. • Develop individualized service plans that identify goals, barriers, resources, and action steps. • Provide ongoing case management and supportive services to participants. • Process funding requests and ensure expenditures comply with program guidelines and funding requirements. • Track, monitor, and manage participant funding allocations and expenditures. • Maintain accurate financial records, invoices, receipts, and supporting documentation related to participant assistance. • Coordinate and monitor educational assistance, housing support, transportation assistance, employment supports, and other approved services. • Maintain regular contact with participants to assess progress, identify barriers, and ensure successful service engagement. • Connect participants to community resources and supportive services. • Assist participants in navigating educational systems, workforce development programs, housing resources, healthcare systems, and other community services. • Monitor participant progress toward educational, employment, housing, and personal development goals. • Track program outcomes, participant engagement, service utilization, and performance measures. • Complete case notes, contact notes, funding documentation, and all required program records according to documentation standards. • Assist with grant reporting, data collection, audits, and quality assurance activities. • Conduct outreach and marketing activities to promote program awareness and participant engagement. • Develop and maintain relationships with referral sources, educational institutions, employers, housing providers, community agencies, and stakeholders. • Represent the program at community meetings, outreach events, resource fairs, and stakeholder meetings. • Participate in supervision, staff meetings, training, and professional development activities. • Complete ongoing reviews and audits of participant files and documentation to ensure compliance. • Follow all policies and procedures for the program and organization. • Perform other duties as assigned by program leadership to support organizational and programmatic goals. Collaborative Partnerships • Participate in team meetings related to consumers receiving services. • Collaborate with internal CHRIS 180 programs to ensure coordinated service delivery. • Work with external agencies and providers to support comprehensive care for consumers. • Make appropriate referrals to community resources addressing health, education, employment, and social service needs. • Attend required meetings including RBWO meetings and Child and Family Team Meetings (CFTM), or provide written updates when necessary. • Advocate for consumers and families when coordinating services with external organizations. Documentation and Compliance • Maintain accurate and timely documentation of services in accordance with CHRIS 180 policies and regulatory requirements. • Complete billing documentation and service notes as required. • Maintain detailed client files in compliance with organizational and state standards. • Protect the confidentiality of consumers and families at all times. • Adhere to CHRIS 180 Code of Conduct, Code of Ethics, and applicable Georgia regulations. • Maintain all required certifications and training and submit documentation to Human Resources as required. Community-Based Service Delivery • Provide services in community settings including homes, schools, neighborhoods, and public housing communities. • Support outreach and engagement efforts within the community. • Maintain flexibility in providing services across programs, grants, and contracts including MAAC, PSSF, CCFA, SOC, and Wraparound services.
