    "ExternalDescriptionStr": "<p style=\"margin-left:0in\"><strong>We are seeking a Seasonal Outpatient Physical Therapist to join our great in-house team!</strong><br><strong>Enjoy flexible scheduling; you choose your availability, ranging from 2–4 days per week.&nbsp;&nbsp;</strong></p>\n<p><strong>Pay Rate: $42 - $68 per hour (depending on experience)</strong><br>\n &nbsp;</p>\n<p><strong>This outpatient role has:</strong></p>\n<ul>\n <li><strong>Flexible schedule - </strong>Achieve work-life balance on your terms</li>\n <li>One-on-one patient care - <strong>No double booking, ever.</strong>&nbsp;</li>\n <li>No administrative hurdles - <strong>Primarily Medicare Part B with no units, no visit limits, no prior authorizations and a full-time rehab coordinator</strong> to handle of all your paperwork/scheduling/etc.&nbsp;</li>\n <li><strong>Diverse caseload</strong> - High-level gait/balance, neurological, orthopedic, and vestibular populations</li>\n <li>Cross-setting flexibility - <strong>Option for both outpatient and inpatient as desired</strong></li>\n <li>Community engagement - <strong>Opportunities to present on wellness topics</strong> and collaborate with local health/wellness professionals as well as program development</li>\n</ul>\n<p>&nbsp;</p>\n<p><strong>What you'll do:</strong></p>\n<p>•&nbsp;&nbsp;&nbsp;&nbsp;Responsible for the maintenance and accuracy of records in regard to patient care for all patients on personal case load.<br>\n •&nbsp;&nbsp;&nbsp;&nbsp;Responsible for the correct reporting of billing information of Physical Therapy in regard to all patients on personal case load.<br>\n •&nbsp;&nbsp;&nbsp;&nbsp;Provide direct patient care as prescribed, including evaluation and treatment planning.<br>\n •&nbsp;&nbsp;&nbsp;&nbsp;Responsible for the reporting and interpretation of treatment results.<br>\n •&nbsp;&nbsp;&nbsp;&nbsp;Participate in development and presentation of in-services for the facility as requested.<br>\n •&nbsp;&nbsp;&nbsp;&nbsp;Supervise support personnel in immediate work areas as appropriate.<br>\n •&nbsp;&nbsp;&nbsp;&nbsp;Perform clinical functions: screens, evaluations, development of treatment plans, and delivery of treatment in accordance with facility, Medicare, and Facility’s policies and procedures.<br>\n •&nbsp;&nbsp;&nbsp;&nbsp;Assist and assess transfers, gait and balance as required by functional mobility of patient level of assistance from minimum to maximum.<br>\n •&nbsp;&nbsp;&nbsp;&nbsp;Manual manipulation, including pushing and/or pulling of patient body parts and limbs.<br>\n •&nbsp;&nbsp;&nbsp;&nbsp;Participate in meetings regarding patient care as appropriate.<br>\n •&nbsp;&nbsp;&nbsp;&nbsp;Assist when appropriate with any community education and public relations activities of the facility.<br>\n •&nbsp;&nbsp;&nbsp;&nbsp;Maintains a high standard of resident care. &nbsp;<br>\n •&nbsp;&nbsp;&nbsp;&nbsp;Responds immediately to all crisis and/or calls for assistance.<br>\n •&nbsp;&nbsp;&nbsp;&nbsp;Actively participates in in-services, department meetings and continuing education.<br>\n •&nbsp;&nbsp;&nbsp;&nbsp;Provide education to patient’s family as well as staff on the specific treatment plan as well as therapy goals for the patient.<br>\n •&nbsp;&nbsp;&nbsp;&nbsp;Responsible for the coordination and cooperation with other staff members in carrying out the treatment plan as established.<br>\n &nbsp;</p>\n<p><strong>You will enjoy:</strong></p>\n<ul>\n <li>401K + employer match</li>\n <li>Employee Assistance Program</li>\n <li>Annual Merit Increase</li>\n <li>$4 Daily Lunch Specials</li>\n <li>Comprehensive orientation with ongoing support and professional development</li>\n <li>Awesome work environments and teams!</li>\n</ul>\n<p>&nbsp;</p>\n<p><strong>Thinking about future growth?</strong><br>\n LCS is the second-largest senior living operator in the nation, with over 149 communities in 33 states… and growing!
