    "description": "<h2>Overview</h2>\n<p>Engages patients and healthcare professionals to efficiently and effectively maximize therapeutic outcomes and is responsible for one or more of the following tasks: dispenses medications, monitors patients' drug therapies, works under collaborative practice agreement or per protocol, and provides consultation and education to patients and other staff.</p>\n<p> </p>\n<p>This is a Full Time, 40 hour per week, benefitted position. Monday-Friday 8am - 4:30pm</p>\n<p> </p>\n<p>Dartmouth is the best where it matters most!</p>\n<p> </p>\n<p>DH Offers You: Employees hired to work at least 0.50 FTE (20 hours per week) and their eligible dependents may participate in Dartmouth Health benefit plans on the employee’s first day of employment.</p>\n<p> </p>\n<p>Medical Insurance: Two medical plans available through Anthem BC/BS:</p>\n<p>High Deductible Health Plan (HDHP) that is matched with a Health Savings Account and a PPO Plan that features higher premiums balanced by copays and lower deductibles.</p>\n<p>Retirement: Employee contributions are vested immediately. Employees also receive a base employer contribution of 4% of biweekly pay plus a</p>\n<p>biweekly matching contribution of 50% on up to 4% of pay saved—for a maximum match of 2% of pay.</p>\n<p>Tuition Reimbursement: Up to $3,000 per year.</p>\n<p>Paid Time Off: Time Off is accrued per pay period based on hours worked. This includes six paid holidays.</p>\n<p>Dental</p>\n<p>Vision</p>\n<p> </p>\n<p>#Dartmouth-Hitchcock Pharmacy, #Pharmacist</p>\n<h2>Responsibilities</h2>\n<li>Gathers a complete medication history and reconciles medication lists as applicable and appropriate.</li>\n<li>Completes a comprehensive medication review to make evidence-based recommendations to ensure medications are necessary, safe and cost effective.</li>\n<li>Identifies and records suspected adverse drug reactions, adherence gaps to medications and barriers to patient adherence.</li>\n<li>If not practicing under a collaborative practice agreement or an approved protocol, contacts providers when there appears to be a possibility of unsafe or inappropriate medication use due to factors including, but not limited to: allergy, medical condition, drug interactions, dosing, and method of administration. Accurately documents all clinical activities and interventions in the electronic record.</li>\n<li>Serves as a consultation resource to medical professionals and answers inquiries on drug therapy and drug products.</li>\n<li>Consults patients and caretakers on all aspects of medication therapy when necessary.</li>\n<li>Assists in discharge planning when requested. Counsels patients and caretakers on proper drug administration, potential drug side effects, interactions and precautions. Helps locate medication supplies not generally available.</li>\n<li>Contributes to the development, maintenance and management of medication related guidelines defined by protocol or collaborative practice agreements.</li>\n<li>Detects and reports medication errors and near misses.</li>\n<li>Ensures readiness for regulatory and accreditation surveys.</li>\n<li>Participates in the quality improvement and medication use review activities of the department.</li>\n<li>Prepares and delivers presentations to facilitate learning or disseminate evidence-based best practice to pharmacists, nurses, physicians, other healthcare professionals in the hospital or ambulatory/outpatient settings.</li>\n<li>Utilizes a solution based approach to ensure heightened patient satisfaction and care.</li>\n<li>Anticipates patient needs and proactively identify solutions.</li>\n<li>Communicates effectively between departments and teams to ensure optimal patient outcomes.</li>\n<li>Actively participates in driving all programs and initiatives to improve patient quality of care, medication compliance, medication adherence and overall efficiency.</li>\n<li>Pursues and maintains certifications necessary to remain compliant with changes to system / department accreditations and standards.</li>\n<li>Participates in the ordering and receiving of controlled substances and other medications as appropriate.</li>\n<li>Effectively utilizes and supervise technicians to maintain an accurate and efficient pharmacy system. Re-direct staff when necessary to ensure service performance goals are met.</li>\n<li>Uses professional judgment to resolve complaints. Act as pharmacist-in-charge in the absence of pharmacy managers and supervisors.</li>\n<li>Outpatient:</li>\n<li>Performs all duties of a registered pharmacist in the state of NH including providing vaccinations approved for administration by the NH Board of Pharmacy. This requirement causes a regular, or frequent potential occupational exposure to blood or bodily fluids and is assigned a Level 1 Classification for occupational exposure to bloodborne pathogens. The employee may be expected to render medical assistance as part of their job description. Bloodborne pathogen training is required and hepatitis B vaccinations will be offered.</li>\n<li>Performs other duties as assigned.</li>\n<h2>Qualifications</h2>\n<ul>\n <li>Graduate of an ACPE-accredited School of Pharmacy with a Bachelor's of Science in Pharmacy</li>\n <li>Doctor of Pharmacy (Pharm D) degree preferred.</li>\n <li>Completion of a post-graduate residency and/or board certified preferred.</li>\n <li>Must be able to work independently with minimal direct supervision, to set priorities, make critical decisions, respond quickly to emergency requests, handle frequent interruptions, and adapt to changes in workload.</li>\n <li>Maintains customer-service focus to achieve excellent working relationships.</li>\n <li>Strong written and verbal communication skills are essential.</li>\n <li>Must be currently licensed to practice in the state of NH, and be willing to seek license reciprocity in multiple states if the position requires it.</li>\n</ul>\n<h2>Required Licensure/Certifications</h2>\n<ul>\n <li>Must have a current, unrestricted pharmacist licensed to practice in the state of NH.</li>\n <li>Board Certification in Specialty Pharmacy (CSP) within 3 years of hire for Clinical Pharmacists in the Specialty Pharmacy, unless designated as part-time or per diem staff.</li>\n <li>BLS Certification within 30 days of hire in the Retail & Specialty Pharmacy if required to administer immunizations</li>\n</ul>",
