You will be responsible for a team of multidiscipline professionals who provide patient care. Specifically, you will be responsible for:</span></p><ul><li dir=\"auto\"><span style=\"color: rgb(43,33,24);background-color: rgb(255,255,255);font-size: 18px;font-family: Noto Sans;\">Analyzing patients&rsquo; conditions, situations, and needs, in conjunction the supervising physician</span></li><li dir=\"auto\"><span style=\"color: rgb(43,33,24);background-color: rgb(255,255,255);font-size: 18px;font-family: Noto Sans;\">Providing high-quality nursing care through supervision of on-site caregivers, case conferences, home assessments, diagnoses care plans, and team exits.</span></li><li dir=\"auto\"><span style=\"color: rgb(43,33,24);background-color: rgb(255,255,255);font-size: 18px;font-family: Noto Sans;\">Evaluating, planning for, and recommending field staffing requirements in accordance with agency requirements.</span></li><li dir=\"auto\"><span style=\"color: rgb(43,33,24);background-color: rgb(255,255,255);font-size: 18px;font-family: Noto Sans;\">Ensuring that each nurse, aide, medical social worker, and rehabilitation specialists works within guidelines of our agency</span></li><li dir=\"auto\"><span style=\"color: rgb(43,33,24);background-color: rgb(255,255,255);font-size: 18px;font-family: Noto Sans;\">Acting as mediator with patients, physicians, case managers, families, and staff</span></li></ul><p style=\"margin-left:0px;\"><span style=\"color: rgb(43,33,24);background-color: rgb(255,255,255);font-size: 18px;font-family: Noto Sans;\">You will be accountable to the Director of Nursing.</span></p><p style=\"margin-left:0px;\"><span style=\"color: rgb(43,33,24);background-color: rgb(255,255,255);font-size: 18px;font-family: Noto Sans;\">You must have a current valid Registered Nurse license and have at least five years&rsquo; experience in clinical care in a health care setting, including managerial experience. &nbsp;<strong>Home Health or Hospice experience a Plus !!!!</strong></span></p><p style=\"margin-left:0px;\"><span style=\"color: rgb(43,33,24);background-color: rgb(255,255,255);font-size: 18px;font-family: Noto Sans;\">You must have excellent nursing skills, possess a high level of initiative, be responsible, and have good communication skills.
