    "requisitionDescription": "<div><div><p><strong><u>Locations</u></strong>: &nbsp;Split: &nbsp;Cooper Clinic, Commerce Clinic</p><p id=\"isPasted\"><strong><u>Purpose of Position:</u></strong></p><p>The Licensed Clinical Social Worker is responsible for integrated behavioral health services in compliance with applicable policies and procedures and other outside regulatory laws, regulations, policies, and procedures. &nbsp;This position conducts mental health assessments, develops integrated treatment plans, makes treatment referrals and conducts crisis interventions. Provides brief counseling for adults, adolescents, children and families and consults with patient&#39;s provider as a member of the multi-disciplinary health care team.&nbsp;</p><p>&nbsp;</p><p><strong><u>JOB DUTIES AND RESPONSIBILITIES:</u></strong></p><p><strong>&nbsp;</strong></p><p><strong>Licensed Clinical Social Worker Duties:</strong></p><ul type=\"disc\"><li>Initial screening and intake (mental health assessment) to determine appropriate level of mental health care. &nbsp;Accurately record patient mental health history, progress notes, and referrals in medical record according to protocol.</li><li>Consult with primary health care clinicians regarding the treatment of mental disorders.</li><li>Assist patients in managing a chronic medical condition or an invasive procedure.</li><li>Assist in the detection of &ldquo;at risk&rdquo; patients and prevent further Psychiatric or physical deterioration.</li><li>Consult with primary care team as necessary and refer cases to specialty Clinicians as appropriate.</li><li>Work with primary care team to effectively and efficiently treat, manage, and counsel patients with chronic emotional and/or health problems.</li><li>Assist patients in prevention of relapse in conditions that tend to become chronic.</li><li>Evaluate patient care plans with primary care team.</li><li>Provide educational information and strategies to staff and patients for mental health management, coping, and self-care.</li><li>Improve clinical outcomes with high prevalence of mental disorders. &nbsp;</li><li>Identify problems related to patient services and make recommendations for improvement.</li><li>Participate in evaluation of peers.</li><li>Participate in patient case review process.</li><li>Attend and participate in meetings and Quality Improvement activities as required.</li></ul><p>&nbsp;</p><p><strong><br>&nbsp;</strong></p><p><strong>Other Duties:</strong></p><ul type=\"disc\"><li>Conduct clinical interviews with clients and relatives in order to elicit information regarding attitudes, social background, developmental history, and other psychiatrically relevant material.</li><li>Assist clients in exploring and gaining an insight into their personality and emotional difficulty, helping them resolve their conflicts, and arriving at more effective ways of coping with their environment. &nbsp;</li><li>Determine, administer, and score the appropriate tests and diagnostic procedures to be used. &nbsp;</li><li>Establish a good working relationship with clients. &nbsp;</li><li>Attend and address meetings of professional groups.</li><li>Give expert court testimony in cases involving mental deficiency or illness commitment proceeding.</li></ul><p>&nbsp;</p><h1>General Duties:</h1><ul><li>Fosters an environment that promotes trust and cooperation among all staff.</li><li>Enforces clinic policies and procedures.</li><li>Maintains confidentiality of all patient and employee information.</li><li>Attends all mandatory meetings and other meetings as requested.</li></ul><p>&nbsp;</p><p><strong>Customer Service:</strong></p><ul><li>Participates in customer service related issues.</li><li>Provides quality care and strives to continually improve.</li><li>Treats patients and others with respect, courtesy and compassion.</li><li>Identifies problems, takes ownership, and contributes to solutions.</li><li>Promotes teamwork and cooperation to create an environment that maximizes efficiency and satisfaction.</li><li>Encourages open communication and mutual education.</li></ul><p><strong><u>&nbsp;</u></strong></p><p><strong><u>KNOWLEDGE, SKILLS, AND ABILITIES:</u></strong></p><p><strong><u>&nbsp;</u></strong></p><p><em>Required</em></p><ul type=\"disc\"><li>Ability to read, write, understand medical terminology</li><li>Knowledge of public health principles and practice, primarily health data privacy and security standards</li><li>Thorough knowledge of maternal and child health, parenting issues, communication skills, family dynamics, and substance abuse</li></ul><ul><li>Excellent interpersonal and written communication skills</li></ul><ul type=\"disc\"><li>Ability to plan and organize work with minimal supervision</li></ul><ul><li>Ability to work well with all staff, community, and volunteers, including mentoring and supervising social work students</li></ul><p>&nbsp;</p><p><em>Preferred</em></p><ul type=\"disc\"><li>Bilingual in English and Spanish is preferred but not required.</li></ul><p>&nbsp;</p><p><strong><u>Credentials and Experience</u></strong></p><p><u>&nbsp;</u></p><p><em>Required</em></p><ul type=\"disc\"><li>Masters in Social Work from an accredited Council on Social Work Education (CSWE) program</li><li>Licensed to practice in the State of Texas, as a Licensed Clinical Social Worker&nbsp;</li><li>Valid Driver&rsquo;s License in the State of Texas</li><li>Certified in Cardiopulmonary Resuscitation (CPR)</li></ul><p>&nbsp;</p><p>This job description shall include, but is not necessarily limited to, the above duties. &nbsp;May temporarily perform other duties as assigned to maintain operations and services.</p><p>&nbsp;</p><p><strong><u>Supervision of Personnel:</u></strong></p><p>N/A</p></div></div>\n",
