This role includes patient education, arranging follow-up services, and ensuring compliance with nursing standards and regulations. By facilitating a smooth transition, the Discharge Planner RN enhances patient satisfaction and improves health outcomes.</p>\n<h2><strong>Key Responsibilities</strong></h2>\n<ul>\n <li><strong>Clinical Review & Documentation</strong>: Conduct clinical reviews of off-site observations, outpatient, and acute care, ensuring accurate documentation in the COREMR System per department guidelines.</li>\n <li><strong>Care Coordination & Management</strong>: Collaborate with Regional and Site Medical Directors, providers, and operations personnel to coordinate care, manage medical costs, and ensure timely accruals and statistics.</li>\n <li><strong>Discharge Planning & Patient Education</strong>: Assess patient needs, develop individualized care plans, educate patients on post-discharge care, review medications, and coordinate follow-up appointments with LVNs.</li>\n <li><strong>Regulatory Compliance & Quality Improvement</strong>: Maintain knowledge of corporate policies, ensure compliance with regulations, participate in quality improvement initiatives, and advocate for medical necessity in community programs.</li>\n <li><strong>Communication & Collaboration</strong>: Establish positive relationships with hospital departments, community agencies, institutional staff, and re-entry services to facilitate effective transitions and continuity of care.</li>\n</ul>\n<h2><strong>Additional Details</strong></h2>M-F 7AM to 3PM \n<h2><strong>Qualifications & Requirements</strong></h2>\n<p>Education</p>\n<ul>\n <li>BSN from an accredited institution</li>\n</ul>\n<p>Experience</p>\n<ul>\n <li>Two (2) years’ clinical experience with Discharge Coordination experience, preferably in acute care settings or correctional settings.</li>\n <li>Previous correctional/detention facility experience</li>\n</ul>\n<p>Licenses/Certifications</p>\n<ul>\n <li>Unrestricted, current RN license in state of employment</li>\n <li>Must maintain all certifications, educational requirements, licensing, etc. for RN license</li>\n <li>Certification in case management or discharge planning, is a plus</li>\n</ul>\n<h2><strong>We are an Equal Employment Opportunity Employer</strong></h2>\n<p>We are committed to fostering, cultivating, and preserving a culture of uniqueness.</p>\n<p> </p>\n<p>We celebrate a variety of backgrounds and are committed to creating an inclusive environment for all employees.</p>\n<p> </p>\n<p><strong>We encourage you to apply!</strong> If you are excited about a role but your experience doesn’t seem to align perfectly with every element of the job description, we encourage you to apply.
