    "description": "<p><u><strong>SUMMARY</strong></u></p>\r\n\r\n<p>The Case Manager for the Enhanced Forensic Respite Bed (EFRB) Program plays a vital role in providing comprehensive case management services to individuals aged eighteen (18) or older with serious and persistent psychiatric disorders, or co-occurring serious psychiatric disorders and substance use disorders, who are involved in active criminal court cases where competence to stand trial may be questioned. This position involves coordinating admissions, providing transportation, facilitating access to housing resources, and developing discharge plans to support successful community reintegration.</p>\r\n\r\n<p>In accordance with RNP&#39;s Mission Statement, Philosophy of Client Care, and Code of Ethics as stated in the Personnel Policies and under the supervision of the Clinical Coordinator, manage client care for individuals requiring multiple assistance sources by performing the following duties.</p>\r\n\r\n<p>&nbsp;</p>\r\n\r\n<p><strong>HOURS: </strong>Sunday - Thursday 4:00 pm - 12:00 am&nbsp;</p>\r\n\r\n<p>&nbsp;</p>\r\n\r\n<p><strong><u>CORE FUNCTIONS</u></strong></p>\r\n\r\n<ul>\r\n\t<li>Provide case Management services in the person&rsquo;s environment of choice with the focus on supporting the person in everyday life based on his/her values and goals, and integration in the community.</li>\r\n\t<li>Provide services in assessment, planning, advocacy, linking, supporting, and monitoring of the activities focused on the individual needs of the persons served in the community with an emphasize on basic survival needs such as housing, income, health, and community resources.</li>\r\n\t<li>Assess and document significant medical, psychological, and social factors contributing to person served and inform other service providers involved in a timely manner to coordinate services.</li>\r\n\t<li>Record adjustments and progress in each individual chart regarding finance, employment, housing, nutrition, clothing, physical and mental health well being.</li>\r\n\t<li>Record and monitor the Residential Rehabilitation/Recovery plans and perform follow up services to determine quality and quantity of service provided to the persons served.</li>\r\n\t<li>Establish and maintain cooperative relationships with community resources, service</li>\r\n\t<li>providers and other organizations.</li>\r\n\t<li>Advocate on behalf of persons served in receiving the required services to become successful and satisfied in the community.</li>\r\n\t<li>Attempt to redirect any person in a respectful and appropriate manner as needed and offer supportive counseling; utilize crisis intervention techniques and contingency plans whenever necessary based on the person&rsquo;s needs and staff assessment.</li>\r\n\t<li>Assist the persons served with maintaining a healthy environment, continuing a daily routine and (re)connecting with natural support systems in the community.</li>\r\n\t<li>Identify appropriate aftercare plan and provide recommendations to Jail Diversion team for court approval.</li>\r\n\t<li>Arrange transportation as needed.</li>\r\n\t<li>Assumes all other appropriate duties as deemed necessary by the supervisor.</li>\r\n</ul>\r\n",
