The Case Manager will engage patients in their treatment, continuing care planning and discharge planning. This role requires knowledge of and experience with community resources and referral systems and/or the ability to work in collaboration with community-based providers and agencies. This role requires basic knowledge of addiction, and mental health recovery processes, services, and systems of support. The Case Manager will promote recovery through role-modeling and respectful interactions with patients and colleagues.&nbsp;</span></p><p><span style=\"font-size: 14px;\"><strong>Pay Structure:</strong></span></p><table border=\"0\" cellpadding=\"0\" cellspacing=\"0\" width=\"324\" data-pasted=\"true\" style=\"width: 25%;\"><colgroup><col style=\"width: 69.969%;\"><col style=\"width: 30.031%;\"></colgroup><tbody><tr><td height=\"20\" class=\"xl63\">Non Masters</td><td class=\"xl63\">$24.00 - $25.00</td></tr><tr><td height=\"20\" class=\"xl63\">Masters non licensed or LCSW</td><td class=\"xl63\">$26.00 - $27.00</td></tr><tr><td height=\"20\" class=\"xl63\">Masters licensed (LICSW or LMHC)</td><td class=\"xl63\">$28.00 - $30.00</td></tr></tbody></table><h1><span style=\"font-size: 14px;\">ESSENTIAL JOB FUNCTIONS/DUTIES/RESPONSIBILITIES:</span></h1><ul><li style=\"font-size: 14px;\">Meet with individual caseload daily to assess further needs and progress in treatment, completing daily contact notes.</li><li style=\"font-size: 14px;\">Conduct patient biopsychosocial and Columbia assessments within 48 hours of patient admittance.&nbsp;</li><li style=\"font-size: 14px;\">Prepare treatment plans and discharge planning in collaboration with patients and their identified supports.</li><li style=\"font-size: 14px;\">Facilitate groups as assigned.&nbsp;</li><li style=\"font-size: 14px;\">Participate in staff supervision, in-service training, and continuing education necessary for certification requirements.</li><li style=\"font-size: 14px;\">Manage caseload and track multiple required tasks using Electronic Health Record.</li><li style=\"font-size: 14px;\">Communicate regularly with referral sources and advocacy groups.</li><li style=\"font-size: 14px;\">Participate in continuing education necessary for certification requirements.&nbsp;</li><li style=\"font-size: 14px;\">As directed under supervision, interact with the financial and utilization management staff for reimbursement.&nbsp;</li><li style=\"font-size: 14px;\">Complete required documentation in accordance with Gosnold policy and regulatory standards.&nbsp;</li><li style=\"font-size: 14px;\">Abide by Gosnold professional code of ethics including those applicable to confidentiality, boundaries, dual relationships, and professional stature as a Gosnold representative.&nbsp;</li><li style=\"font-size: 14px;\">Communicate regularly with referral sources and advocacy groups.</li><li style=\"font-size: 14px;\">Must possess skills in customer service orientation.</li><li style=\"font-size: 14px;\">Perform all other duties as assigned.</li></ul><p><span style=\"font-size: 14px;\"><strong>MINIMUM KNOWLEDGE AND SKILLS REQUIRED:</strong></span></p><p><span style=\"font-size: 14px;\"><em>The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job.&nbsp;</em></span></p><ul><li style=\"font-size: 14px;\"><strong>Education:&nbsp;</strong>Bachelor&rsquo;s degree in a healthcare or related discipline is preferred.<strong>&nbsp;</strong></li><li style=\"font-size: 14px;\"><strong>Experience:&nbsp;</strong>Minimum one (1) year of experience in working with mental health, addiction and/or home health/hospice or related field will be considered for this position.
