    "description": "<p><span style=\"font-family:Arial,Helvetica,sans-serif\"><strong><u>SUMMARY</u></strong></span></p>\n\n<p><span style=\"font-family:Arial,Helvetica,sans-serif\">The Case Manager role involves coordinating admissions, providing transportation, facilitating access to housing resources, and developing discharge plans to support successful community reintegration.</span></p>\n\n<p><span style=\"font-family:Arial,Helvetica,sans-serif\">In accordance with RNP&#39;s Mission Statement, Philosophy of Client Care and Code of Ethics as stated in the Personnel Policies and under the supervision of the Clinical Coordinator, manage client care for individuals requiring multiple assistance sources by performing the following duties:</span></p>\n\n<p>&nbsp;</p>\n\n<p><u><strong>HOURS</strong></u><strong>&nbsp;: (2 shifts available)</strong></p>\n\n<p>Sunday - Thursday 8:00 am- 4:00 pm&nbsp;</p>\n\n<p>Sunday - Thursday 4:00 pm - 12:00 am</p>\n\n<p>&nbsp;</p>\n\n<p><span style=\"font-family:Arial,Helvetica,sans-serif\"><strong><u>CORE FUNCTIONS</u></strong></span></p>\n\n<ul>\n\t<li><span style=\"font-family:Arial,Helvetica,sans-serif\">Provide case Management services in the person&rsquo;s environment of choice with the focus on supporting the person in everyday life based on his/her values and goals, and integration in the community.</span></li>\n</ul>\n\n<ul>\n\t<li><span style=\"font-family:Arial,Helvetica,sans-serif\">Provide services in assessment, planning, advocacy, linking, supporting, and monitoring of the activities focused on the individual needs of the persons served in the community with an emphasize on basic survival needs such as housing, income, health, and community resources.</span></li>\n\t<li><span style=\"font-family:Arial,Helvetica,sans-serif\">Assess and document significant medical, psychological, and social factors contributing to the person served and inform other service providers involved in a timely manner to coordinate services.</span></li>\n\t<li><span style=\"font-family:Arial,Helvetica,sans-serif\">Record adjustments and progress in each individual chart regarding finance, employment, housing, nutrition, clothing, and physical and mental health well-being.</span></li>\n\t<li><span style=\"font-family:Arial,Helvetica,sans-serif\">Record and monitor the Residential Rehabilitation/Recovery plans and perform follow-up services to determine the quality and quantity of service provided to the persons served.</span></li>\n\t<li><span style=\"font-family:Arial,Helvetica,sans-serif\">Establish and maintain cooperative relationships with community resources, service&nbsp;providers, and other organizations.</span></li>\n\t<li><span style=\"font-family:Arial,Helvetica,sans-serif\">Advocate on behalf of persons served in receiving the required services to become successful and satisfied in the community</span></li>\n\t<li><span style=\"font-family:Arial,Helvetica,sans-serif\">Attempt to redirect any person in a respectful and appropriate manner as needed and offer supportive counseling; utilize crisis intervention techniques and contingency plans whenever necessary based on the person&rsquo;s needs and staff assessment.</span></li>\n\t<li><span style=\"font-family:Arial,Helvetica,sans-serif\">Assist the persons served in maintaining a healthy environment, continuing a daily routine, and (re)connecting with natural support systems in the community.</span></li>\n\t<li><span style=\"font-family:Arial,Helvetica,sans-serif\">Identify appropriate aftercare plan and provide recommendations to the Jail Diversion team for court approval.</span></li>\n\t<li><span style=\"font-family:Arial,Helvetica,sans-serif\">Arrange transportation as needed.</span></li>\n\t<li><span style=\"font-family:Arial,Helvetica,sans-serif\">Assumes all other appropriate duties as deemed necessary by the supervisor.</span></li>\n</ul>\n\n<p>&nbsp;</p>\n",
