    "ExternalDescriptionStr": "<p>The Inpatient Medical Coder II is responsible for coding and applying ICD-10-CM and PCS codes as applicable to code medical records for Brooks Rehabilitation Hospital.&nbsp;Reviews data from the medical&nbsp;record to determine or confirm codes. Performs analysis of physician documentation and provides&nbsp;feedback for improvement. Collaborates with internal and external resources to obtain additional documentation to support the services provided, documentation and codes billed.</p>\n<p><strong>Responsibilities:&nbsp;</strong></p>\n<ul>\n <li>Reviews medical record to correctly apply and/or validate ICD-10-CM IRF-PAI codes.</li>\n <li>Supports timely, accurate and complete documentation of clinical information, facilitating&nbsp;modifications&nbsp;to&nbsp;clinical&nbsp;documentation&nbsp;to&nbsp;support&nbsp;services rendered&nbsp;and&nbsp;reimbursement&nbsp;received.</li>\n <li>Maintains knowledge&nbsp;of&nbsp;coding&nbsp;rules&nbsp;and&nbsp;regulations&nbsp;by&nbsp;staying&nbsp;current on&nbsp;issues&nbsp;regarding&nbsp;medical&nbsp;coding, compliance and&nbsp;reimbursement.</li>\n <li>Ability to accurately assign the IGC, etiologic diagnosis, and principal diagnosis for the UB04 and all applicable comorbidities, complications, and procedure codes</li>\n <li>Maintains&nbsp;coding accuracy&nbsp;of 95%&nbsp;or&nbsp;above&nbsp;for&nbsp;assigned&nbsp;codes.</li>\n <li>Completes the coding and data entry within four days of receipt and follows standard coding&nbsp;protocols&nbsp;for&nbsp;appropriate assignment&nbsp;of&nbsp;diagnoses&nbsp;and procedures.</li>\n <li>Seeks&nbsp;clarification&nbsp;for&nbsp;missing&nbsp;or&nbsp;inadequate&nbsp;information&nbsp;needed&nbsp;for&nbsp;accurate code&nbsp;assignment&nbsp;from appropriate resources.</li>\n <li>Maintains&nbsp;individual&nbsp;production&nbsp;defined&nbsp;by&nbsp;productivity&nbsp;standards&nbsp;with&nbsp;minimal&nbsp;supervision.</li>\n <li>Examines&nbsp;records timely;&nbsp;reviewing&nbsp;principal&nbsp;and&nbsp;secondary&nbsp;diagnoses and&nbsp;procedures</li>\n <li>Identifies&nbsp;records with&nbsp;opportunities&nbsp;for&nbsp;improved&nbsp;documentation.</li>\n <li>Communicates&nbsp;with&nbsp;designated&nbsp;staff&nbsp;either&nbsp;directly or&nbsp;through&nbsp;queries to&nbsp;facilitate&nbsp;complete and accurate documentation.</li>\n <li>Provides&nbsp;feedback regarding&nbsp;current&nbsp;coding practices&nbsp;and&nbsp;changes in&nbsp;regulations&nbsp;and&nbsp;guidelines&nbsp;to&nbsp;improve&nbsp;the accuracy&nbsp;of&nbsp;final code&nbsp;assignment</li>\n <li>Assists in maintaining accounts receivables at minimal levels as defined by departmental&nbsp;protocol&nbsp;by&nbsp;accurately&nbsp;assessing&nbsp;and&nbsp;correcting&nbsp;issues regarding medical&nbsp;necessity,&nbsp;claims denials,&nbsp;bundling&nbsp;issue sand&nbsp;charge capture.</li>\n <li>Efficiently&nbsp;uses&nbsp;available&nbsp;reference and&nbsp;coding&nbsp;tools and&nbsp;third&nbsp;party&nbsp;payer&nbsp;resources&nbsp;to&nbsp;research,&nbsp;maintain knowledge&nbsp;and&nbsp;provide feedback&nbsp;to&nbsp;department.</li>\n</ul>\n<p><strong>Qualifications:&nbsp;</strong></p>\n<ul>\n <li>Associate or Bachelor’s Degree, preferred but not required</li>\n <li>A minimum of 5 years of coding experience as a certified medical coder</li>\n <li>Inpatient Rehabilitation experience preferred but not required</li>\n</ul>\n<p><strong>Hours:</strong>&nbsp;40 Hours per week, Monday - Friday&nbsp;</p>\n<p><strong>This is a Remote position.
