Under the observation of their supervising Physician and respective department Chief/Director, the APRN provides extraordinary patient care throughout BHA Clinics.</p><h1><u>GENERAL EDUCATION REQUIREMENTS</u></h1><p>Master of Science in Nursing from an educational institution approved by the Florida Department of Health.</p><ul><li>Clear and active Florida Advanced Practice Registered Nurse&nbsp;licensure</li><li>3+ years&rsquo; experience of related clinical experience preferred. Experience in a primary health care environment, community health center or Federally Qualified Health Center (FQHC)&nbsp;preferred</li><li>Experience working in various outpatient&nbsp;settings</li><li>Current Basic Life Support (BLS)/CPR certification from the American Heart&nbsp;Association or the American Red Cross</li><li>Proficient in Electronic Medical Records&nbsp;(EMR)</li><li>Demonstrates strong oral and written communication&nbsp;skills</li><li>Ability to speak and understand the English language to effectively communicate with&nbsp;patients</li><li>Extensive knowledge of physical assessment, differential diagnosis, pathophysiology</li></ul><h1><u>PRIMARY ACCOUNTABILITIES</u></h1><ul><li>Manages acute, episodic outpatient care and health maintenance needs through the provision of&nbsp;a Primary Care Medical&nbsp;Home</li><li>Demonstrates competent skills in therapeutic, cost-effective pharmacologic administration&nbsp;and management</li><li>Develops and implements formal and informal teaching plans for patients, families, staff, and community &nbsp;</li><li>Promotes patient/family-informed decision making in treatment plans and&nbsp;options</li><li>Performs physicals, diagnostic tests, and screening&nbsp;evaluations</li><li>Manages overall patient care regarding lifestyle and development&nbsp;issues</li><li>Emphasizes preventative care and disease&nbsp;management</li><li>Understands the changes in health promotion throughout the aging process, consults with the collaborating physician as&nbsp;warranted</li><li>Collaborates with respective department Chief/Director in development of clinical protocols/procedures&nbsp;for delivery of outpatient&nbsp;care</li><li>Provides care to patients in a safe environment</li><li>Participates in departmental Continuous Quality Improvement efforts and provides support for improvement of identified issues</li><li>Works with clinical manager to ensure collaborative problem solving with staff through participation in identification, mediation, and resolution of issues and team problems</li><li>Promotes staff and community enhancement through coordination of in-services to&nbsp;staff, Physicians, and partners to promote profession and&nbsp;organization</li><li>Attends monthly department staff meetings, Provider meetings, and other scheduled&nbsp;meetings</li><li>Maintains Quality Department Performance&nbsp;measures</li><li>Maintains predetermined productivity based on patient&nbsp;volumes</li><li>Maintains internal customer&nbsp;satisfaction</li><li>Takes ownership and represents Brevard Health Alliance&rsquo;s mission and organizational goals established by the executive committee. &nbsp;These include, but are not limited to: Productivity, Quality, Patient Satisfaction, and Employee Retention. &nbsp;It is the duty of each BHA employee to incorporate these goals into all daily work routines and strive to achieve and surpass all goals by working as a team. &nbsp;The mission and goals of BHA will be incorporated into each associate&rsquo;s Key Performance Indicators and will be evaluated on a monthly and quarterly basis.&nbsp;</li></ul></div></div></div></div>\n",
