We offer competitive pay, great benefits with a culture to match.</em></p><p><strong>POSITION SUMMARY</strong></p><p>The LPN provides nursing care to CCHS patients, assisting physicians and nurse practitioners with various aspects of patient care and performing patient testing, as ordered. The LPN helps to ensure smooth patient flow through the clinic and also assists with some clerical responsibilities.</p><p><strong>KEY RESPONSIBILITIES</strong></p><p>1.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Assists as needed with initiation of medical history and physical assessment. Draws blood and collects other specimens for lab analysis. Prepares for and assists with medical procedures such as GYN exams, wound care, etc.</p><p>2.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Provides telephone triage and initiates follow-up calls directed by the physician or clinic charge nurse. Refers disposition to supervisor and documents in medical record the results of phone call.</p><p>3.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Educates patients and families within the scope of knowledge base; provides informational material as appropriate.&nbsp;</p><p>4.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Assists with staff education and supervision such as preceptorship.</p><p>5.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Responds to inquiries, communicating policies, procedures, and standards to patients, families, visitors, and fellow employees.</p><p>6.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Cleans exam and treatment rooms following patient discharge, appropriately disposing of trash to provide a clean environment; wipes up spills and sanitizes area, as needed.</p><p>7.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Maintains adequate level of supplies, medications, linens, and equipment, requisitioning additional supplies, as needed.</p><p>8.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Ensures patient care equipment is clean and properly functioning, reporting defective equipment and supplies to supervisor.</p><p>9.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Assists with quality improvement activities as directed</p><p>10.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Complete tasks for medication refills as needed for the assigned care team.</p><p>11.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Implement and maintain morning team huddles</p><p>12.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Follow up on STD tracking report for assigned care team</p><p>13.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Oversee VFC program</p><p>14.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Follow up on patients with abnormal labs by calling patients</p><p>15.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Perform daily quality control checks on appropriate clinic equipment and document in log records</p><p>16.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Assist as needed with same day referral request</p><p>17.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>Uphold and implement Nursing standing orders and protocols</p><p>18.<span style=\"white-space:pre;\">&nbsp; &nbsp;&nbsp;</span>May perform other duties as required.</p><p>JOBS THIS POSITION DIRECTLY SUPERVISES</p><p>Depending upon work site, the LPN may supervise one or more of the following jobs.</p><p><strong>POSITION REQUIREMENTS</strong></p><p>Education: High school diploma plus specialized training.</p><p>Experience: Minimum of one year nursing experience</p><p>Licenses or Certifications: LPNTN license is required.
