The CHW will work in tandem with a Public Health Nurse to provide personalized, community-based care to these clients. The CHW will assist clients in finding community resources which reduce their barriers to care and help ensure that the client is knowledgeable about their health and the health of their infant(s). This position is grant funded through a Community Partnership Grant. </p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Duties Include:</span></p>\n<p><br></p>\n<p>•    Client outreach and enrollment activities with pregnant and postpartum individuals.<br>•    Conducts home and community visits with families and infants from diverse cultural, socioeconomic, educational, racial, and ethnic backgrounds.<br>•    Identifies needs based on the social determinants of health and provides appropriate resources, referrals and assistance.<br>•    Educates clients about the importance of preventative health care; provides basic education to ensure an understanding of their health status and conditions.<br>•    Accurately documents all client contact in an Electronic Health Record System including using mobile technology for partial remote work and on home visits.<br>•    Attends local events and disseminates health related information throughout the community.<br>•    Other related duties as assigned including participating in emergency preparedness or response activities.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Requirements:</span></p>\n<p><br><br></p>\n<p><span style=\"font-weight: bold\">Community Health Worker in Training</span></p>\n<p><br><br></p>\n<p>•    High school diploma or equivalent<br>•    Must be certified as a Community Health Worker within the first year of employment<br>•    Ohio Driver's License<br>•    Bilingual communication preferred. <br>•    Experience in community health coordination/navigation preferred.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Community Health Worker I</span></p>\n<p><br></p>\n<p>•    High school diploma or equivalent<br>•    Knowledge and proficiency using an Electronic Health Record system<br>•    Community Health Worker Certification<br>•    Ohio Driver's License<br>•    Bilingual communication preferred. <br>•    Experience working in community health, social work, public health or community education preferred.<br>•    P&amp;W: OBGYN, prenatal and/or postpartum experience preferred.</p>\n<p><br></p>\n<p>   <span style=\"font-weight: bold\">Community Health Worker II</span></p>\n<p><br></p>\n<p>•    High school diploma or equivalent<br>•    3 years’ experience as Community Health Worker or working in community health, public health, or community education setting.<br>•    Knowledge and proficiency using an Electronic Health Record system<br>•    Community Health Worker Certification<br>•    Ohio Driver's License<br>•    Bilingual communication preferred. <br>•    Associate's degree in public health, public administration, social work or other human services related field preferred.<br>•    P&amp;W: OBGYN, prenatal and/or postpartum experience preferred</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">Hiring Salary: </span></p>\n<p><br><br></p>\n<p>Community Health Worker in Training: $19.05/hour to $21.91/hour. <br>Community Health Worker I: $20.30/hour to $23.35/hour.<br>Community Health Worker II: $22.63/hour to $26.03/hour.</p>\n<p>This is a non-exempt position.</p>\n<p><br><span style=\"font-weight: bold\">Interested applicants should apply at https://franklincounty.bamboohr.com/jobs/ with: </span></p>\n<p>•    Resume<br>•    Cover letter <br>•    Completed FCPH application (located: <a href=\"http://www.myfcph.org/careers)\" target=\"_blank\" rel=\"noopener noreferrer\">http://www.myfcph.org/careers)</a> </p>\n<p><br></p>\n<p>Deadline for Applying: Internal applicants (06/25/2026); External applicants (Until Filled)      </p>\n<p><br></p>\n<p><span style=\"font-weight: bold\">No phone calls please. </span> </p>\n<p>This is a grant-funded position and is contingent upon funding availability.</p>\n<p><br></p>\n<p>Franklin County Public Health is proud to be an Equal Employment Opportunity employer.
