This shelter is located in Long Island City, NY. (Queens)</span></p><p style='margin:0in;font-size:16px;font-family:\"Times New Roman\",serif;'><span style=\"font-size: 14px; font-family: georgia, serif;\">&nbsp;</span></p><p style='margin:0in;font-size:16px;font-family:\"Times New Roman\",serif;'><span style=\"font-size: 14px; font-family: georgia, serif;\">&nbsp;</span></p><p style='margin:0in;font-size:16px;font-family:\"Times New Roman\",serif;'><span style=\"font-size: 14px; font-family: georgia, serif;\"><strong>PRIMARY RESPONSIBILITIES</strong></span></p><p style='margin:0in;font-size:16px;font-family:\"Times New Roman\",serif;'><span style=\"font-size: 14px; font-family: georgia, serif;\"><strong>&nbsp;</strong></span></p><ul style=\"list-style-type: disc;margin-left:undefined;\"><li style=\"font-size: 14px; font-family: georgia, serif;\"><span style=\"line-height: 103%; color: black;\">Ensure document folder in CARES (VDP, Client rules and regulations, Crib acknowledgment, ACS Video, etc.) is completed and documented immediately upon client arrival</span></li><li style=\"font-size: 14px; font-family: georgia, serif;\"><span style=\"line-height: 103%; color: black;\">Provide field orientation and on the job training to Case Managers</span></li><li style=\"font-size: 14px; font-family: georgia, serif;\"><span style=\"line-height: 103%; color: black;\">Create and execute plans for meeting contracted ILP completion and placement goals</span></li><li style=\"font-size: 14px; font-family: georgia, serif;\"><span style=\"line-height: 103%; color: black;\">Ensure all Case Managers complete mandated CRF, DHS, and OTDA tasks in accordance with prescribed timeframes and mandates</span></li><li style=\"font-size: 14px; font-family: georgia, serif;\"><span style=\"line-height: 103%; color: black;\">Ensure Case Managers are providing consistent services in accordance CRF, DHS and OTDA policies and procedures</span></li><li style=\"font-size: 14px; font-family: georgia, serif;\"><span style=\"line-height: 103%; color: black;\">Monitor Case Managers work activities via CARES reviews, physical chart reviews, and spreadsheets</span></li><li style=\"font-size: 14px; font-family: georgia, serif;\"><span style=\"line-height: 103%; color: black;\">Conduct individual and group supervisions with Case Managers. Provide staff with documented feedback of performance, including areas of strength and areas in need of development</span></li><li style=\"font-size: 14px; font-family: georgia, serif;\"><span style=\"line-height: 103%; color: black;\">Conduct performance evaluations of Case Managers as per CRF, DHS, and OTDA policies and professional standards</span></li><li style=\"font-size: 14px; font-family: georgia, serif;\"><span style=\"line-height: 103%; color: black;\">Collaborate with Social Services team to develop service plans to address clients&rsquo; needs related to housing, substance abuse treatment, mental health and/or employment, etc</span></li><li style=\"font-size: 14px; font-family: georgia, serif;\"><span style=\"line-height: 103%; color: black;\">Meet with Senior Management (PD and APD) for individual and group supervision</span></li><li style=\"font-size: 14px; font-family: georgia, serif;\"><span style=\"line-height: 103%; color: black;\">Assist families with identifying appropriate housing resources and assist application process as needed</span></li><li style=\"font-size: 14px; font-family: georgia, serif;\"><span style=\"line-height: 103%; color: black;\">Monitor demographic report to better guide Case Managers and ensure program compliance with CRF, DHS, and OTDA expectations</span></li><li style=\"font-size: 14px; font-family: georgia, serif;\"><span style=\"line-height: 103%; color: black;\">Attend and moderate Case Conferences with Case Managers and challenging, noncompliant and/or housing ready clients</span></li><li style=\"font-size: 14px; font-family: georgia, serif;\"><span style=\"line-height: 103%; color: black;\">Establish linkage with community service providers to obtain housing, health, and social services for clients</span></li><li style=\"font-size: 14px; font-family: georgia, serif;\"><span style=\"line-height: 103%;
