This position provides case management, therapeutic support, advocacy, and community-based services to individuals living with serious mental illness, substance use disorders, and other complex health challenges.</p><p style=\"margin-left:0in;\">The ideal candidate is someone who values relationship-building, meets people where they are, and is passionate about helping individuals navigate pathways toward stability, wellness, and housing.</p><p style=\"margin-left:0in;\"><strong>What You Will Do</strong></p><div style=\"margin-left:0in;\"><ul style=\"margin-left: 0in;\"><li style=\"margin-left:0in;\">Provide outreach, case management, counseling, advocacy, and other needed services to clients seeking services in Center.&nbsp;</li><li style=\"margin-left:0in;\">Collaborate with client in the formulation of an Individual Recovery Plan (IRP) that is reviewed and modified with client on a regular basis;</li><li style=\"margin-left:0in;\">Provide therapeutic counseling and support services to each client in a broad range of areas, including (but not limited to) housing, training &amp; work opportunities, activities of daily living, health, wellness self-management, relapse prevention, self-help and empowerment, problem solving, family life and social relationships and integrated treatment for substance abuse;</li><li style=\"margin-left:0in;\">Coordinate and monitor referrals to community services, and advocate client&rsquo;s participation in them.
