The Registered Nurse (RN) will provide support to the health, wellness, and disease management of HealthFirst Patients.</p><p><br></p><p><strong>DUTIES and RESPONSIBILITIES:</strong></p><p>The Registered Nurse (RN) is responsible for nursing care at patient appointments</p><ul><li>Obtaining vital signs and patient history</li><li>Providing individualized education to patients related to acute and chronic diagnosis</li><li>Triaging patient calls and schedules visits as appropriate</li><li>Administering oral and injectable medication and treatments within the scope of practice and with orders from providers. May distribute sample medication per provider orders.</li><li>May perform venipuncture blood draws and urine collection for lab work as ordered by the provider</li><li>Maintaining patient data and health information in the electronic medical record according to documentation guidelines</li><li>Generating patient-related reports, as needed</li><li>Participating in performance improvement programs and disease management programs.</li><li>Assisting in the coordination of well-child and immunization programs</li><li>Assisting in ordering and stocking clinical supplies</li><li>Completing transitional care management follow-up for patients with hospital admissions</li><li>Monitoring assigned AthenaOne Buckets for patient follow-up according to outline protocol, including patient portal messages.</li><li>Monthly participation on the Life Risk Safety Committee</li><li>Routine safety and risk assessment to support Just Culture.</li><li>Supporting care coordination efforts as requested by the primary care provider</li></ul>",
