This role provides the opportunity to deliver skilled nursing care in a supportive, home-like environment LPNs are responsible for planning, implementing, and evaluating resident care to ensure the highest standards of quality, safety, and regulatory compliance are consistently met.</p>\n<h3>Available Shifts</h3>\n<ul>\n <li>Day Shift 7am - 3pm </li>\n <li>Evening Shift 3pm -11pm </li>\n <li>Overnight Shift 11pm- 7am<em>(Availability may vary for Full-Time, Part-Time, and Per Diem roles — please inquire during application.)</em></li>\n</ul>\n<p>New graduates are welcome. Elderwood proudly supports nurses transitioning from student to licensed professional through hands-on mentorship, continuing education, and a collaborative team environment designed to support growth and confidence.</p>\n<p> </p>\n<p>Take the next step in your nursing journey with a team where your skills, compassion, and leadership make a meaningful impact every day.</p>\n<p> </p>\n<p>Apply today - we look forward to meeting you!</p>\n<h2>Responsibilities</h2>\n<p><strong>LPN - Licensed Practical Nurse: </strong></p>\n<ul>\n <li><p>Administer medications and treatments to assigned residents as ordered by physician and utilizing the five rights as applicable.</p></li>\n <li><p>Direct and monitor the personal care duties and nursing care procedures carried out by Nursing Assistants of the assigned team.</p></li>\n <li><p>Assist Nursing Assistants with direct care of residents, as necessary.</p></li>\n <li><p>Report to Unit Manager/Assistant Unit Manager or Charge Nurse Supervisor unusual symptoms, changes of condition, daily needs, and progress of residents.</p></li>\n <li><p>Follow the Plan of Care for each resident, and monitor the performance of Nursing Assistants in implementation of the care plan.</p></li>\n <li><p>Receive controlled substances from the shift Supervisory Nurse and is responsible for all aspects of the storage and distribution of these medications; ensure proper and safe placement and use of medications stored in medication cart, stock medication cupboards, medication room refrigerator and narcotic cabinets.</p></li>\n <li><p>Document as applicable in the care plans, Nursing Notes, Medical Administration Records, Treatment Records and other required records.</p></li>\n</ul>\n<h2>From</h2>\n<h2>Up to</h2>\n<h2>Qualifications</h2>\n<p><strong>LPN - Licensed Practical Nurse:</strong></p>\n<ul>\n <li><p>Credentials as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) with a Vermont (VT) license required. </p></li>\n <li><p>LPN Training and work experience in a geriatric setting preferred. </p></li>\n <li><p>Ability to supervise and work well with other personnel required. </p></li>\n <li><p>Ability to communicate well verbally and in writing required. </p></li>\n <li><p>Caring, respectful attitude towards residents required. </p></li>\n <li><p>Physical stamina for constant activity required. </p></li>\n <li>Ability to perform tasks to established standards of excellence required.</li>\n <li>This position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
