Every day, our teams serve hundreds of individuals across multiple levels of care — many arriving in crisis and leaving with stability, skills, and hope.</p><p>As a <strong>Case Manager / Group Facilitator</strong>, you will be part of that impact — helping individuals move from crisis to stability and from isolation to connection.</p><p>This role combines <strong>group facilitation and case management</strong>, allowing you to build strong connections with clients while supporting their overall treatment journey.</p><p>You will plan and lead groups, support individual client needs, and collaborate with a multidisciplinary team to ensure clients are progressing and receiving the appropriate level of care.</p><p>This position is ideal for someone who is <strong>engaging, organized, and passionate about helping others grow in a structured, supportive environment with Group-heavy position serving two programs, with case management.</strong></p><p><br></p><p>&nbsp;<strong>Schedule:</strong> Monday – Friday | 8:30 AM – 5:00 PM &amp; 11:30 AM - 8:00 PM</p><p><br></p><p><strong>ESSENTIAL FUNCTIONS:</strong></p><p><strong>1.</strong> &nbsp;<strong>Group Facilitation &amp; Client Engagement</strong></p><ul><li>&nbsp;80–90% of this position involves group facilitation, plan, organize, and facilitate psychoeducational and process groups using evidence-based practices and program curriculum</li><li>Create a safe, inclusive, and supportive group environment that fosters participation and growth</li><li>Manage group dynamics and navigate challenges with professionalism and confidence</li><li>Provide individual check-ins, support, and crisis intervention as needed</li></ul><p><strong>2.</strong> &nbsp;<strong>Case Management &amp; Care Coordination</strong></p><ul><li>Support clients with resource coordination, goal setting, and treatment engagement</li><li>Assist with discharge planning and continuity of care</li><li>Act as a liaison between clients, clinical staff, and external providers</li><li>Help remove barriers to care and support overall client progress</li></ul><p><strong>3.</strong> &nbsp;<strong>Treatment Collaboration</strong></p><ul><li>Collaborate with clinical staff to align group content and case management with treatment plans</li><li>Participate in multidisciplinary team meetings and case discussions</li><li>Contribute to treatment planning and client progress reviews</li></ul><p><strong>4.</strong> &nbsp;<strong>Documentation &amp; Compliance</strong></p><ul><li>Maintain accurate and timely documentation including group notes, attendance, and case management updates</li><li>Ensure compliance with confidentiality standards, ethical guidelines, and regulatory requirements</li></ul><p><strong>5.</strong> &nbsp;<strong>Program Development &amp; Growth</strong></p><ul><li>Support development and improvement of group curriculum and programming</li><li>Incorporate community resources and creative approaches into services</li><li>Participate in ongoing training, supervision, and professional development</li></ul><p><strong>ADDITIONAL FUNCTIONS:</strong></p><ol><li>Performs other duties as may be assigned.</li><li>Follows and supports TPEG policies and procedures.</li><li>Works collaboratively and cooperatively with internal and external partners.</li><li>Represents TPEG, Inc., by embodying the mission, vision and values of the organization.</li><li>Travels occasionally during the workday and on occasional overnight stay.
