    "requisitionDescription": "<div><div><div><p><br></p><p id=\"isPasted\"><strong><span style=\"font-size: 16px;\">Home Care Intake Specialist</span></strong>&nbsp;$28.58/hour</p><p><strong>&nbsp;</strong></p><p><strong>Position Summary:&nbsp;</strong>This position is responsible for initial client assessments, service planning and coordination of care in compliance with AGE and agency guidelines within the 15 communities</p><p>serviced by Bristol Aging &amp; Wellness, Inc.</p><p>&nbsp;</p><p><strong>Principal Accountabilities:&nbsp;</strong></p><ul><li>Assures that clients are assessed on a timely basis.</li><li>Conducts a comprehensive evaluation to determine the full scope of clients&rsquo; needs.&nbsp;</li><li>Determines eligibility for programs.&nbsp;</li><li>Develops appropriate service plans that meet consumer choice and budget guidelines.&nbsp;</li><li>Partners with vendors to implement home care services.&nbsp;</li><li>Assists clients with problem solving and makes referrals as needed.&nbsp;</li><li>Determines eligibility for public benefits.&nbsp;</li><li>Assists with applications for public benefits and health insurance and problem resolution related to benefits.&nbsp;</li><li>Documents phone calls, visits, and all activity in accordance with Elder Affairs&rsquo; and agency requirements.&nbsp;</li><li>Maintains client records accurately and within the mandated timeframe.&nbsp;</li><li>Communicate with case management staff upon a case transfer.</li></ul><p>&nbsp;</p><p><strong>Qualifications:&nbsp;</strong></p><p><u>Education</u>: bachelor&rsquo;s degree in human services or related discipline.&nbsp;</p><p><u>Experience</u>: Elder service helpful.&nbsp;</p><p><u>Special Skills/Knowledge</u><u>:</u>&nbsp;</p><ul><li>Excellent oral and written communications skills,&nbsp;</li><li>ability to effectively relate to a wide variety of personalities,&nbsp;</li><li>strong assessment skills,&nbsp;</li><li>basic computer skills,&nbsp;</li><li>organizational and time management skills,&nbsp;</li><li>ability to multi-task,&nbsp;</li><li>be adaptable and work in a fast-paced, high volume work environment.&nbsp;</li><li>Driver&rsquo;s license and reliable transportation required.&nbsp;</li></ul><p>&nbsp;</p><p>This job description is intended to be general, will evolve over time, and is subject to periodic updating.</p><p>&nbsp;</p><p>This job description does not constitute a written or implied contract of employment.</p><p>EOE/M/F/D/V</p><p><br></p></div></div></div>\n",
